Blood after bowel movements?

Hello, doctor: I experienced a sharp pain in my anus while having a bowel movement, and I noticed some spots of blood on the toilet paper.
What could this indicate? For the past few days, I have also been experiencing intermittent abdominal discomfort after bowel movements, and sometimes there are small amounts of white and black substances in my stool.
Could this be a sign of colorectal cancer? Given my history of abdominal surgery, is it safe for me to undergo a colonoscopy?

Ss, 10~19 year old female. Ask Date: 2016/02/21

Dear Ms.
Ss, based on your symptoms, you may have an anal fissure.
It is recommended that you drink plenty of water and eat fruits.
If the symptoms persist, please consult a gastroenterologist or a colorectal surgeon.

More Info

Blood after a bowel movement can be concerning, and it’s important to understand the potential causes and when to seek medical help. The presence of blood, especially if it is bright red, often indicates that the source of bleeding is in the lower gastrointestinal tract, such as the rectum or anus. Common causes of rectal bleeding include hemorrhoids, anal fissures, diverticular disease, and inflammatory bowel diseases like ulcerative colitis or Crohn's disease. In some cases, more serious conditions, such as colorectal cancer, can also present with blood in the stool.

Given your symptoms of sharp pain during bowel movements and the presence of blood on the toilet paper, it is possible that you may be experiencing an anal fissure or hemorrhoids. An anal fissure is a small tear in the lining of the anus, often caused by passing hard stools, which can lead to sharp pain and bleeding. Hemorrhoids, which are swollen veins in the rectal area, can also cause similar symptoms.
The additional symptoms you mentioned, such as abdominal discomfort and the presence of white and black substances in your stool, warrant further investigation. The white material could be mucus, which is often associated with inflammatory bowel conditions, while black material could indicate the presence of digested blood, which is a sign of bleeding higher up in the gastrointestinal tract.
Regarding your concern about colorectal cancer, while it is a possibility, especially given your history of abdominal surgery, it is essential to note that many benign conditions can also cause similar symptoms. However, any new or unusual gastrointestinal symptoms, especially those involving blood, should be evaluated by a healthcare professional.

As for your previous abdominal surgery, whether you can undergo a colonoscopy depends on the type of surgery and your current health status. Generally, colonoscopy is a safe procedure, but it is crucial to discuss your surgical history with your doctor to ensure that it is appropriate for you. They may recommend imaging studies or other diagnostic tests before proceeding with a colonoscopy.

You should seek medical attention promptly, especially since you have experienced ongoing symptoms. A healthcare provider will likely perform a physical examination, possibly including a digital rectal exam, and may recommend further testing such as a colonoscopy or imaging studies to determine the cause of your symptoms.
In summary, while there are several potential causes for the symptoms you are experiencing, it is essential to consult with a healthcare professional for an accurate diagnosis and appropriate treatment. Early intervention can be crucial in managing any underlying conditions effectively.
