Right testicular pressure pain?
Hello, I would like to ask about pain I experience when lightly pressing on the upper and inner side of my right testicle (while standing).
What could be the possible causes? Recently, I have not experienced any trauma, and the left side is normal without any pain.
Sometimes, my urine has a strong odor and I notice a splitting phenomenon.

Right testicular pain can be concerning and may arise from various causes, some of which may require medical attention. Given your description of pain in the upper part of the right testicle, particularly when pressed, and the additional symptoms of a foul odor in your urine and a splitting phenomenon during urination, it is essential to consider several potential underlying issues.
1. Epididymitis: This is an inflammation of the epididymis, the tube that stores and carries sperm. It can cause localized pain and tenderness in the testicle, often accompanied by swelling. Epididymitis can be caused by infections, including sexually transmitted infections (STIs) or urinary tract infections (UTIs). The foul odor in your urine could suggest an infection, which might be related to this condition.
2. Orchitis: This is an inflammation of the testicle itself, which can occur due to viral or bacterial infections. Like epididymitis, orchitis can cause pain and swelling in the affected testicle.
3. Testicular Torsion: Although you mentioned that there was no trauma, testicular torsion is a medical emergency that involves the twisting of the spermatic cord, cutting off blood supply to the testicle. This condition typically presents with sudden, severe pain and swelling and requires immediate medical intervention. If the pain worsens or is accompanied by nausea or vomiting, seek emergency care.
4. Hernia: An inguinal hernia occurs when tissue protrudes through a weak spot in the abdominal muscles. This can sometimes cause discomfort or pain in the groin or testicular area, especially when standing or lifting.
5. Varicocele: This is an enlargement of the veins within the scrotum and can lead to a dull ache or discomfort in the testicular area. It is often more noticeable when standing or during physical activity.
6. Kidney Stones: Although the pain is localized to the testicle, kidney stones can cause referred pain to the testicular area. The foul odor in your urine could also be indicative of a urinary tract issue related to kidney stones.
7. Urinary Tract Infection (UTI): The presence of a foul odor in your urine may suggest a UTI, which can sometimes cause referred pain to the testicular area. Symptoms of a UTI can include burning during urination, increased frequency, and urgency.
Given your symptoms, it is crucial to consult a healthcare professional for a thorough evaluation. A physical examination, along with a urinalysis and possibly an ultrasound of the scrotum, may be necessary to determine the exact cause of your pain. Early diagnosis and treatment are essential, especially if an infection or torsion is involved.
In the meantime, avoid any activities that may exacerbate the pain, such as heavy lifting or strenuous exercise. Over-the-counter pain relief, such as ibuprofen or acetaminophen, may help alleviate discomfort, but it is not a substitute for professional medical advice.
In summary, while there are several potential causes for your right testicular pain, it is essential to seek medical attention to rule out serious conditions and receive appropriate treatment. Your health and well-being are paramount, and timely intervention can prevent complications.
