Low sperm count?

Dear Doctor: I had a sperm analysis done in 2011, which showed a count of 31 million.
However, I had another sperm analysis yesterday, and the count was less than 1 million, so motility could not even be assessed.
What could be the reasons for such a significant drop in sperm count? Recently, I have also noticed a tingling sensation in my testicles.
Thank you, Doctor.

Victor, 30~39 year old female. Ask Date: 2016/03/10

Dr. Fu Shizhang reply Urology

There are many possible reasons for a decrease in sperm count; however, if it is accompanied by testicular pain, the most likely cause is varicocele.
It is recommended to visit a urology clinic for an examination.

Low sperm count, or oligospermia, is a significant concern for many men, especially when it comes to fertility. The drastic drop from 31 million sperm per milliliter to less than 1 million is alarming and warrants a thorough investigation. Several factors can contribute to such a decline in sperm count, and understanding these can help address potential health issues and fertility concerns.

Possible Causes of Low Sperm Count
1. Medical Conditions: Various medical conditions can affect sperm production. One common condition is varicocele, which is the enlargement of veins within the scrotum. This can lead to increased temperature in the testicles, adversely affecting sperm production. The "acidic" sensation you mentioned in your testicles could be related to this condition or other underlying issues.

2. Hormonal Imbalances: Hormones play a crucial role in sperm production. Disorders of the hypothalamus, pituitary gland, or testicles can lead to hormonal imbalances that affect sperm production. Conditions such as Klinefelter syndrome or other genetic disorders can also impact hormone levels.

3. Infections: Infections in the reproductive tract can lead to inflammation and affect sperm production. Conditions like epididymitis or orchitis can cause pain and swelling in the testicles, which may also contribute to a lower sperm count.

4. Environmental Factors: Exposure to environmental toxins, such as pesticides, heavy metals, and industrial chemicals, can negatively impact sperm production. Lifestyle factors, including smoking, excessive alcohol consumption, and drug use, can also contribute to low sperm counts.

5. Lifestyle Choices: Obesity, poor diet, lack of exercise, and high-stress levels can all negatively affect sperm production. Maintaining a healthy lifestyle is crucial for optimal reproductive health.

6. Heat Exposure: Prolonged exposure to heat, such as from hot baths, saunas, or tight-fitting clothing, can impair sperm production. The testicles need to be slightly cooler than body temperature for optimal sperm production.

Implications for Fertility
A sperm count of less than 1 million is significantly below the normal range, which is generally considered to be above 15 million sperm per milliliter. This low count can severely impact fertility, making it more challenging to conceive. However, it is essential to note that sperm quality is also determined by motility (the ability to swim) and morphology (the shape of the sperm). Even with a low count, if the sperm that are present are of good quality, there may still be a chance for conception.

Next Steps
Given the significant drop in your sperm count and the discomfort you are experiencing, it is crucial to consult a urologist or a fertility specialist. They can perform a comprehensive evaluation, including:
- Physical Examination: To check for any abnormalities in the reproductive organs.

- Hormonal Testing: To assess hormone levels that may be affecting sperm production.

- Ultrasound: To check for conditions like varicocele or other structural issues.

- Repeat Semen Analysis: To confirm the initial findings and assess sperm quality.

Conclusion
Low sperm count can be a complex issue influenced by various medical, environmental, and lifestyle factors. Early intervention is key to addressing any underlying conditions and improving fertility outcomes. If you are planning to conceive, it is advisable to seek medical advice promptly to explore treatment options and lifestyle modifications that can enhance your reproductive health.
