Scars: Can They Really Fade and How to Treat Them? - Plastic Surgery

Scars cannot be completely eliminated?

Hello, about six months ago I experienced a minor car accident that left a scar on my leg.
Since then, I have been applying scar cream (polyxal) almost every day, but it seems to have improved only to a certain extent.
Occasionally, the scar itches, and I can't help but scratch it.
What does this mean? Is the scar cream still effective? Will it eventually fade? Can the scar be removed in the future? Thank you.

Z, 10~19 year old female. Ask Date: 2016/03/12

Dr. Lu Daokuan reply Plastic Surgery

There are two types of scar creams: one is silicone-based, which helps prevent scar formation, and the other is for reducing pigmentation.
Given your current condition, you should consider seeing a plastic surgeon.

Hello! Thank you for your question regarding scars and their treatment. Scars are a natural part of the healing process after an injury, and they can vary in appearance based on several factors, including the type of injury, the location on the body, and individual healing responses.
When you experience a cut or abrasion, your body goes through a complex healing process that involves inflammation, tissue formation, and remodeling. Initially, the scar may appear red, raised, or discolored, but over time, many scars can fade and become less noticeable. However, the extent to which a scar fades can depend on various factors, including genetics, skin type, and the care taken during the healing process.

You mentioned that you have been using a scar cream (polyxal) daily. While topical treatments can be beneficial, their effectiveness can vary. Many over-the-counter products contain ingredients like silicone, which has been shown to help flatten and soften scars. Silicone sheets or gels are often recommended for their effectiveness in scar management. Other ingredients, such as vitamin E, onion extract, or hydroquinone, may also be present in scar treatments, but their efficacy is less consistent.

The itching you experience could be a normal part of the healing process, as the skin undergoes changes. However, it’s essential to avoid scratching the scar, as this can lead to further irritation or even infection, which may worsen the appearance of the scar. If the itching persists or worsens, it might be worth consulting a dermatologist to rule out any underlying issues.

As for whether scars can be completely removed, it’s important to understand that while many scars can fade significantly, complete removal is often not possible. However, there are various treatment options available to improve their appearance:
1. Topical Treatments: As mentioned, silicone gels or sheets can be effective. Other creams may help with pigmentation or texture.

2. Laser Therapy: Laser treatments can target the scar tissue and help reduce redness, flatten raised scars, and improve overall texture.

3. Microneedling: This procedure involves using fine needles to create micro-injuries in the skin, promoting collagen production and potentially improving the appearance of scars.

4. Chemical Peels: These can help with discoloration and texture by removing the outer layer of skin.

5. Surgical Options: In some cases, surgical revision may be an option for more significant scars, where a dermatologist or plastic surgeon can remove the scar tissue and close the wound in a way that minimizes future scarring.

6. Injections: For raised scars (hypertrophic or keloid scars), corticosteroid injections can help flatten the scar.

It's essential to consult with a dermatologist to discuss your specific scar and the best treatment options for you. They can assess the scar's characteristics and recommend a personalized treatment plan.
In conclusion, while scars can fade over time, the degree of fading varies. Consistent care with appropriate products can help improve their appearance, but for more significant results, professional treatments may be necessary. Remember to be patient with the healing process, as it can take time for scars to mature and fade. Thank you for your question, and I hope this information helps you on your journey to scar management!
