Pain in the buttocks during bowel movements?

Hello doctor, I would like to ask you about something.
For the past two days, I have experienced significant pain in my rectum during bowel movements.
The first time I wiped, there was bleeding, but the second time there wasn't.
I'm wondering if this could be due to not drinking enough water recently, as I have had this symptom for over a year.
When I drink plenty of water, I don't experience pain or bleeding during bowel movements.
I'm really scared that I might have colon cancer!

An, 20~29 year old female. Ask Date: 2016/03/13

Painful bowel movements accompanied by bleeding are most commonly caused by hemorrhoids or anal fissures.
It is advisable to pay attention to hydration and fiber intake, and to develop good bowel habits.
If there is frequent pain or bleeding at the anal area, it is recommended to consult a colorectal specialist or a gastroenterologist for evaluation and treatment.

Experiencing painful bowel movements can be distressing, and it's understandable to be concerned about potential underlying issues, including the fear of serious conditions like colorectal cancer. However, it's important to recognize that there are several common and less severe causes of painful bowel movements, particularly when accompanied by symptoms like bleeding.

One of the most frequent causes of pain during bowel movements is constipation, which can lead to hard stools that are difficult to pass. When stool is hard, it can cause anal fissures—small tears in the lining of the anus—which can result in sharp pain and bleeding during and after bowel movements. This is often exacerbated by inadequate hydration and a low-fiber diet, which can lead to harder stools. It sounds like your symptoms have been linked to your hydration levels, as you mentioned that drinking more water tends to alleviate the pain and bleeding.
In addition to constipation and anal fissures, other potential causes of painful bowel movements include hemorrhoids, which are swollen veins in the lower rectum and anus that can cause pain, itching, and bleeding. Hemorrhoids can be internal or external, and they often result from straining during bowel movements, prolonged sitting, or increased pressure during pregnancy or obesity.

Another possibility is a condition known as proctitis, which is inflammation of the lining of the rectum. This can be caused by infections, inflammatory bowel disease (IBD), or radiation therapy. Symptoms may include pain, bleeding, and a feeling of urgency to have a bowel movement.

While your concerns about colorectal cancer are valid, especially given the fear associated with gastrointestinal symptoms, it is crucial to remember that the likelihood of cancer is relatively low in younger individuals without other significant risk factors. Colorectal cancer typically presents with other symptoms such as significant changes in bowel habits, unexplained weight loss, persistent abdominal discomfort, or blood in the stool that is not related to hemorrhoids or fissures.

Given your symptoms have persisted for over a year, it would be advisable to consult with a healthcare professional for a thorough evaluation. They may recommend a physical examination, possibly including a digital rectal exam, and may suggest further diagnostic tests such as a colonoscopy, especially if there are concerning symptoms or if you have a family history of colorectal cancer.

In the meantime, focusing on dietary changes can be beneficial. Increasing your intake of dietary fiber through fruits, vegetables, whole grains, and legumes can help soften stools and promote regular bowel movements. Additionally, staying well-hydrated by drinking plenty of water is essential. Regular physical activity can also aid in digestion and help prevent constipation.

In summary, while painful bowel movements can be alarming, they are often due to benign conditions such as constipation, anal fissures, or hemorrhoids. However, given the duration and recurrence of your symptoms, it is important to seek medical advice to rule out any serious conditions and to receive appropriate treatment. Taking proactive steps in your diet and hydration can significantly improve your symptoms and overall gastrointestinal health.
