The stomach feels a strange sensation of trembling, but there is no pain?

Hello, Doctor.
May I ask you a question? Thank you.
I am 20 years old.
One day, when it was very cold, I suddenly coughed very hard, and it felt like my stomach had a hole or something.
Since that day, sometimes when I cough or breathe, I feel a twitching sensation in my stomach, like a small electric shock, which happens several times, but it doesn't hurt.
It just feels strange.
What could this situation be? Which specialist should I see? I don't have much education, so I apologize for not being able to describe it well.
Thank you.
I have habits of smoking, drinking alcohol, chewing betel nut, staying up late, having an irregular diet, and I often drink sugary beverages but not much plain water.

Dr. Wu Yingxun reply Pulmonology

Hello: It may be that the abdominal muscle spasms are caused by excessive coughing.
You could consult a neurologist.
If there is pain in the upper abdomen, it may be necessary to see a gastroenterologist.

It sounds like you're experiencing some unusual sensations in your stomach, particularly a trembling or shaking feeling that occurs during coughing or deep breathing. This can understandably be concerning, especially since it started after a forceful cough. While I cannot provide a diagnosis, I can help you understand what might be happening and when to seek medical advice.

First, it's important to note that the sensation you describe could be related to several factors. One possibility is that the diaphragm, which is the muscle that separates the chest from the abdomen and plays a crucial role in breathing, might be reacting to the forceful cough. Sometimes, after a strong cough, the muscles in the abdominal area can become tense or spasm, leading to sensations that feel like tremors or vibrations. This is often benign and may resolve on its own.

However, given your lifestyle factors—such as smoking, drinking alcohol, consuming betel nut, irregular sleep patterns, and poor dietary habits—there could be additional considerations. These habits can contribute to gastrointestinal issues, including acid reflux, gastritis, or even functional gastrointestinal disorders, which might explain some of the discomfort you're feeling. The fact that you mentioned feeling like you are "shocked" or "electrified" could also suggest that there is some irritation or sensitivity in the stomach or surrounding areas.

If these sensations persist, worsen, or are accompanied by other symptoms such as pain, nausea, vomiting, or changes in bowel habits, it would be wise to seek medical attention. In your case, starting with a visit to a gastroenterologist would be appropriate. They can conduct a thorough evaluation, which may include a physical examination, a review of your medical history, and possibly imaging studies or endoscopic procedures to assess your gastrointestinal tract.

In addition to seeking medical advice, consider making some lifestyle changes that could improve your overall gastrointestinal health. Here are a few recommendations:
1. Hydration: Increase your water intake. Staying hydrated is crucial for digestive health.

2. Diet: Focus on a balanced diet rich in fruits, vegetables, whole grains, and lean proteins. Try to limit processed foods, excessive caffeine, and alcohol.

3. Smoking Cessation: If possible, consider quitting smoking. It can significantly impact your gastrointestinal health and overall well-being.

4. Regular Sleep: Aim for a consistent sleep schedule. Poor sleep can exacerbate stress and digestive issues.

5. Stress Management: Engage in stress-reducing activities such as exercise, meditation, or yoga, which can help alleviate some gastrointestinal symptoms.

6. Avoiding Triggers: Pay attention to foods or activities that seem to trigger your symptoms and try to avoid them.

In summary, while the trembling sensation in your stomach may not be immediately alarming, it is essential to monitor your symptoms and seek medical advice if they persist or worsen. A gastroenterologist can help determine the underlying cause and recommend appropriate treatment. Taking proactive steps to improve your lifestyle can also contribute positively to your digestive health.
