Which specialty should I see?
Hello Doctor, I have many strange little ailments in my body, and I really don't know which specialty to consult.
I experience hand tremors that are difficult to control; my hands shake when I hold things, when someone touches me, while picking up food, and even when my hands are held up.
The shaking worsens when I'm nervous, but it seems to be less noticeable when I'm moving.
I have seen a neurologist, and after an EEG, the doctor said there were no issues.
I also visited an endocrinologist, had blood tests to rule out hyperthyroidism, and an ultrasound revealed thyroid nodules, but the cause of my tremors remains unidentified.
My blood pressure and heart rate can also spike significantly due to emotional stress.
Additionally, I often have nightmares and wake up remembering them vividly.
Sometimes I wake up in the middle of the night and uncontrollably cry.
My sister, who sleeps with me, says that sometimes I suddenly sit up in bed and laugh softly or speak incoherently before lying back down.
I would like to ask the doctor what might be wrong with me and which specialty I should consult for treatment.

Dear Miss You: Please discuss this with your neurologist; based on your description, I believe it may be a neurological issue.
If necessary, a consultation with a psychiatrist may also be required.
Imaging studies or immunological and biochemical tests may not definitively identify the cause; rather, insights can be gained from clinical presentations and neurological examinations.

Based on your description of various unexplained symptoms, it seems you are experiencing a complex interplay of physical and psychological issues. The symptoms you mentioned, such as uncontrollable hand tremors, emotional dysregulation, and vivid nightmares, can be indicative of several underlying conditions. Given the multifaceted nature of your symptoms, it would be advisable to consult with a few different medical specialties to ensure a comprehensive evaluation and treatment plan.
1. Neurology: Since you have already seen a neurologist, it might be beneficial to return for a follow-up, especially if your symptoms have progressed or changed. Neurologists specialize in disorders of the nervous system, and they can conduct further tests, such as MRI scans or additional neurophysiological assessments, to rule out conditions like essential tremor, Parkinson's disease, or other neurological disorders that could cause tremors and emotional disturbances.
2. Psychiatry: Given your experiences with emotional dysregulation, nightmares, and episodes of sudden laughter or nonsensical speech during sleep, a psychiatrist could provide valuable insights. Conditions such as anxiety disorders, mood disorders, or even sleep disorders like REM sleep behavior disorder could be contributing to your symptoms. A psychiatrist can evaluate your mental health, prescribe medications if necessary, and recommend therapy options such as cognitive-behavioral therapy (CBT) to help manage anxiety and emotional responses.
3. Endocrinology: Although you have already been evaluated for hyperthyroidism, it might be worth revisiting an endocrinologist, especially if there are any other hormonal imbalances that could be affecting your mood and physical symptoms. Hormonal fluctuations can significantly impact both physical and mental health.
4. Sleep Medicine: Since you are experiencing vivid nightmares and episodes of waking confusion, consulting a sleep specialist could be beneficial. They can conduct a sleep study to assess for conditions like sleep apnea or other sleep disorders that could be affecting your overall health and contributing to your daytime symptoms.
5. Psychosomatic Medicine: This specialty focuses on the interaction between psychological and physical health. A physician in this field can help address how stress and emotional factors may be manifesting as physical symptoms, such as tremors and emotional outbursts.
In addition to consulting these specialists, it may also be helpful to keep a detailed diary of your symptoms, noting when they occur, their severity, and any potential triggers. This information can provide valuable insights to your healthcare providers and assist them in making a more accurate diagnosis.
Lastly, consider seeking support from a therapist or counselor who can help you navigate the emotional aspects of your symptoms. They can provide coping strategies and support as you work through this challenging time.
In summary, a multidisciplinary approach involving neurology, psychiatry, endocrinology, sleep medicine, and psychosomatic medicine may be the most effective way to address your complex symptoms. Each specialty can contribute unique insights and treatment options, ultimately leading to a better understanding of your health and improved quality of life.
