A tear in the foreskin, with a particularly tight ring around the foreskin?

Hello Doctor, I would like to briefly describe my condition.
I believe my foreskin is somewhat long, but I am unclear about the medical criteria for determining if it is excessively long.
When fully erect, the tip of the glans can be slightly exposed, but I cannot retract the foreskin completely without external force.
In this state, it feels tight but not painful.
When not erect, the foreskin completely covers the glans, and there is some excess skin at the tip (I apologize if this term is not accurate).
Since my genital and urinary functions are normal, I have never consulted a clinic regarding the length of my foreskin.
A few days ago, I experienced pain while urinating in the morning during an erection and discovered that I had a tear.
Prior to this, there were no unusual symptoms.
Upon closer inspection, I found that there were multiple tears arranged in a circular pattern around the genital area, and I noticed that this circular area felt particularly tight.
Could this condition be due to excessive dryness, or is there another underlying cause?

Possible phimosis with inflammation.
It is recommended to see a urologist for examination.

Understanding tight foreskin and tear injuries is crucial for maintaining genital health and comfort. Based on your description, it seems that you are experiencing a condition known as phimosis, where the foreskin is too tight to be easily retracted over the glans (the head of the penis). This can lead to discomfort, especially during erections or sexual activity, and may result in tears or injuries if the foreskin is forcibly retracted.

Phimosis can be classified into two types: physiological and pathological. Physiological phimosis is common in infants and young boys, as the foreskin is naturally adhered to the glans and typically resolves on its own as the child grows. Pathological phimosis, on the other hand, occurs when the foreskin becomes tight due to scarring, inflammation, or other medical conditions. In your case, the tightness you are experiencing, especially during erections, suggests that it may be pathological.

The tearing you experienced could be attributed to several factors, including:
1. Dryness: Insufficient lubrication can lead to friction, which may cause tearing of the skin. This is particularly common if there is a lack of natural moisture or if the area is not adequately lubricated during sexual activity or masturbation.

2. Tightness of the Foreskin: As you noted, the tightness of the foreskin can lead to discomfort and tearing, especially during erections when the skin is stretched.

3. Infection or Inflammation: Conditions such as balanitis (inflammation of the glans) or posthitis (inflammation of the foreskin) can cause swelling and tightness, leading to tearing.

4. Forcible Retraction: Attempting to retract the foreskin forcibly can lead to tears. It’s important to avoid forcing the foreskin back if it does not retract easily.

To address your situation, consider the following steps:
1. Gentle Stretching: Gradual and gentle stretching of the foreskin may help increase its elasticity over time. This should be done carefully to avoid further tearing.

2. Moisturizing: Applying a water-based lubricant or a gentle moisturizer to the foreskin can help reduce friction and dryness, making it easier to retract without causing injury.

3. Avoiding Forcible Retraction: It’s essential to avoid forcing the foreskin back, especially if it causes pain. This can lead to more severe injuries and complications.

4. Consulting a Healthcare Professional: Given your symptoms and the occurrence of tearing, it would be wise to consult a urologist or a healthcare provider specializing in male genital health. They can assess the tightness of your foreskin and determine if any treatment is necessary. Treatment options may include topical steroids to help with inflammation and tightness, or in some cases, circumcision may be recommended if the condition is severe and persistent.

5. Monitoring for Infection: Keep an eye on the area for signs of infection, such as increased redness, swelling, or discharge. If you notice any of these symptoms, seek medical attention promptly.

In summary, your situation appears to be related to tight foreskin and potential phimosis, which can lead to discomfort and tearing. Addressing dryness, avoiding forcible retraction, and consulting a healthcare professional for a thorough evaluation and treatment plan are essential steps to take. Remember that maintaining open communication with your healthcare provider will help ensure that you receive the best care for your condition.

Hello: It is likely due to repeated small tears, which create a narrow ring at the opening. Therefore, when the glans is retracted, it may be prone to tearing. If the condition worsens, a circumcision is recommended. Wishing you good health.
