Premature ejaculation, sensitivity, acne?

Hello, doctor.

1.
I am 17 years old.
During middle school, I masturbated an average of 3.4 times a week, and in high school, about 1.2 times a week or once every two weeks.
I have noticed that when I watch videos and masturbate, I do not specifically control myself or take breaks before ejaculation; I ejaculate within a minute.
Could this be considered premature ejaculation?
2.
Recently, I started dating a girl.
At first, I would get an erection just from holding hands, but now that we have been together for a while, that doesn't happen anymore.
When we hug or kiss, I notice a clear, sticky fluid coming from the glans, and the amount varies depending on how long we hug or kiss.
The longer we do it, the more fluid there seems to be.
Recently, after touching my girlfriend's breasts, the fluid increased even more.
When I excessively stimulate the area around my testicles, I feel something like a ball, and the pain lasts until I sleep.
Is there any concern with the increased amount of clear, sticky fluid? Can I control its release?
3.
I usually eat fried or spicy foods, and I don't get acne.
However, after being intimate with my girlfriend and experiencing an excessive release of clear fluid, I notice a significant increase in acne the next day.
Similarly, after masturbating, I also see a spike in acne.
I have read various articles online stating that masturbation does not cause acne, but I personally experience an increase in breakouts the day after.
Is there a way to address this issue?

There is no clear correlation between premature ejaculation and the frequency of masturbation or the occurrence of acne.
Currently, the diagnostic definition of premature ejaculation is based on the ejaculation time during actual intercourse, not during masturbation.
For a detailed definition, please refer to my previous article.
When a male is sexually stimulated, the prostate and bulbourethral glands begin to secrete before ejaculation, so the presence of a clear, viscous fluid at the urethral opening is considered normal.
However, if there is any abnormal coloration or accompanying symptoms of urethral pain, it is advisable to seek medical attention to rule out infection issues.

Understanding the concerns surrounding premature ejaculation, sensitivity, and acne in teenagers is crucial for both physical and emotional well-being. Let's address your questions one by one.

1. Premature Ejaculation: At 17 years old, experiencing ejaculation within a minute during masturbation can indeed be concerning, especially if it feels uncontrollable. Premature ejaculation (PE) is defined as ejaculation that occurs sooner than desired, either during sexual intercourse or masturbation. It's important to note that many young men experience this, particularly during adolescence when sexual arousal can be heightened. Factors contributing to PE can include anxiety, excitement, or lack of experience. If this is a persistent issue that affects your sexual experiences or relationships, it might be beneficial to consult a healthcare professional who can provide guidance and potential treatment options.

2. Fluid Discharge: The transparent, sticky fluid you describe is likely pre-ejaculatory fluid, which is normal and serves to lubricate the urethra. The amount can vary based on arousal and stimulation. It’s common for this fluid to increase with heightened sexual activity, such as kissing or touching. If you experience pain or discomfort in the testicular area, it could be due to overstimulation or even a condition known as epididymitis, which is inflammation of the epididymis. If the pain persists or worsens, it’s advisable to seek medical attention. As for controlling the fluid discharge, it’s primarily a natural response to arousal, and while you can try to manage your stimulation levels, it’s not something that can be completely controlled.

3. Acne Concerns: The relationship between sexual activity, masturbation, and acne is a common topic of discussion. While many sources state that masturbation does not cause acne, hormonal fluctuations during puberty can lead to increased oil production in the skin, which can contribute to acne. The stress or anxiety associated with sexual experiences might also exacerbate acne for some individuals. If you notice a pattern where acne appears after sexual activity or masturbation, it might be worth considering your overall skincare routine and lifestyle. Maintaining a consistent skincare regimen, avoiding touching your face, and managing stress through healthy outlets can help. If acne continues to be a significant issue, consulting a dermatologist can provide tailored advice and treatment options.

In summary, your experiences are common among teenagers, and while they can be concerning, they are often part of normal development. If you find that these issues are affecting your quality of life or self-esteem, do not hesitate to reach out to a healthcare professional for support and guidance. They can provide reassurance, education, and potential treatment options tailored to your needs. Remember, open communication with trusted adults or healthcare providers can help you navigate these changes more comfortably.
