Short-term penile engorgement and pain after ejaculation?
Hello, doctor.
After ejaculation (whether through normal intercourse or masturbation), I experience a period of time (up to one day at most) where I feel a throbbing pain during erections, and my penis becomes particularly sensitive for a short time (any contact causes discomfort).
However, when not erect, I do not experience any discomfort.
Is this normal?

More Info
Post-ejaculation pain and sensitivity in men can be a concerning issue, and it’s important to understand the underlying mechanisms and potential causes. What you are experiencing, characterized by a feeling of pain or discomfort in the penis after ejaculation, is not uncommon. Many men report similar symptoms, which can range from mild discomfort to more pronounced pain that may last for varying durations, including up to a day as you mentioned.
Understanding the Symptoms
1. Physiological Response: After ejaculation, the body undergoes a series of physiological changes. The penis becomes less sensitive as the body enters a refractory period, which is the time following orgasm during which a man cannot achieve another erection. During this period, some men may experience heightened sensitivity or discomfort, particularly if there is any physical contact with the penis.
2. Nerve Sensitivity: The nerves in the genital area can become temporarily hypersensitive after ejaculation. This heightened sensitivity can lead to discomfort with touch, which is often described as a burning or aching sensation. This is typically a normal response, but if it persists or worsens, it may warrant further investigation.
3. Pelvic Floor Tension: The pelvic floor muscles play a crucial role in sexual function. After ejaculation, these muscles may remain tense or spasm, leading to discomfort. This can be exacerbated by anxiety or stress, which can cause muscle tension throughout the body, including the pelvic region.
4. Inflammation or Infection: In some cases, post-ejaculation pain can be a sign of inflammation or infection in the prostate (prostatitis) or other parts of the reproductive system. If the pain is severe, persistent, or accompanied by other symptoms such as fever, urinary difficulties, or changes in sexual function, it is essential to seek medical attention.
5. Hormonal Factors: Hormonal fluctuations can also play a role in post-ejaculation sensitivity. Testosterone levels can influence libido and sensitivity, and variations in these levels may contribute to the sensations you are experiencing.
When to Seek Medical Advice
While occasional post-ejaculation discomfort can be normal, you should consider consulting a healthcare professional if:
- The pain is severe or lasts longer than a day.
- You experience additional symptoms such as swelling, redness, or discharge.
- There are changes in your urinary habits or sexual function.
- You have a history of prostate issues or sexually transmitted infections.
Management Strategies
1. Relaxation Techniques: Engaging in relaxation exercises or pelvic floor muscle relaxation can help alleviate tension in the pelvic area. Techniques such as deep breathing, yoga, or meditation may be beneficial.
2. Avoiding Irritation: After ejaculation, consider avoiding any direct stimulation of the penis for a period to allow sensitivity to normalize.
3. Hydration and Diet: Staying well-hydrated and maintaining a balanced diet can support overall health, including sexual health.
4. Consultation with a Specialist: If symptoms persist, consider seeing a urologist or a sexual health specialist. They can conduct a thorough examination and may recommend tests to rule out any underlying conditions.
In conclusion, while post-ejaculation pain and sensitivity can be a normal response for many men, it is essential to monitor the symptoms and seek medical advice if they become bothersome or persistent. Understanding your body’s responses and maintaining open communication with healthcare providers can help ensure your sexual health and overall well-being.
