Suspected excessive masturbation?
Hello, Dr.
Liu: I am currently in my senior year of high school.
During my last masturbation session, it lasted about two hours (though not with continuous erection, as I took breaks in between).
When I finally ejaculated, there was a significant amount of semen, and afterward, I noticed pain in my left testicle, which appeared slightly swollen.
I have experienced similar situations in the past, but after resting for about 1 to 2 hours, everything returned to normal.
Additionally, after this session, I found that my foreskin suddenly "cannot" cover the glans anymore, even when not erect, as it retracts behind the glans.
Previously, when not erect, the foreskin could cover the glans, and during an erection, it would retract slightly, and I could manually pull it back to expose the glans completely.
I would appreciate it if you could answer the following questions:
1.
Is the testicular pain due to prolonged masturbation?
2.
Is there a concern for testicular torsion?
3.
If I hold off on ejaculation during masturbation, will the semen be thinner? (Based on my experience, if I ejaculate quickly, the semen seems to be thicker.)
4.
Why did my foreskin retract behind the glans after this session, and why can't I push it back when not erect? (I could do that before.) Is this normal?
5.
Will exposing the glans lead to decreased sensitivity and gradually reduce the original pleasure?
Thank you!

1.
During the male erection process, contraction of the pelvic muscles is necessary.
Excessive frequency of masturbation may lead to temporary soreness, which will improve after some time.
However, prolonged standing may cause swelling and soreness in the left scrotum, and it is important to rule out the possibility of varicocele.
It is advisable to consult a urologist, as severe cases may affect semen quality.
2.
In cases of testicular torsion, the pain in the testicle will progressively worsen and is unrelated to masturbation; your symptoms do not seem to match this condition.
3.
If the frequency of masturbation is excessive, the semen may appear thinner.
It is recommended to take a break for a few days and then reassess.
4.
If the foreskin retracts after an erection and cannot return, accompanied by persistent swelling and pain in the glans, it is advised to seek medical attention promptly to rule out the possibility of phimosis.
If there are no such symptoms, close observation should be sufficient.
5.
Sensation and the exposure of the glans should not be directly related.
Given the complexity of your symptoms, if they persist, it is essential to seek evaluation and treatment from a qualified physician.

Hello! Thank you for your questions regarding excessive masturbation and the concerns you have about your testicular pain, sensitivity, and changes in your foreskin. Let's address your questions one by one, and I'll provide some additional context based on medical knowledge.
1. Is testicular pain caused by prolonged masturbation?
Testicular pain can occur after prolonged sexual activity, including masturbation. This pain may be due to several factors, including muscle strain, increased blood flow to the area, or even minor trauma. If the pain is mild and resolves after a short period of rest, it is likely not a cause for concern. However, if the pain persists or worsens, it is essential to seek medical attention to rule out any underlying conditions.
2. Is there a concern about testicular torsion?
Testicular torsion is a medical emergency that occurs when the spermatic cord becomes twisted, cutting off blood supply to the testicle. Symptoms include sudden, severe pain, swelling, and sometimes nausea. If you experience severe pain that does not subside, or if you notice any swelling or changes in color, you should seek immediate medical attention. Given that your pain has been mild and resolved after rest, it is less likely to be torsion, but it’s always best to err on the side of caution.
3. Does delaying ejaculation result in thinner semen?
The consistency of semen can vary based on several factors, including the duration of abstinence before ejaculation. Generally, if you wait longer between ejaculations, the semen may appear thicker due to the accumulation of sperm and seminal fluid. Conversely, if you ejaculate frequently, the semen may be thinner. This is a normal physiological response and varies from person to person.
4. Why did my foreskin retract and not cover the glans after this episode? Is this normal?
The retraction of the foreskin can be influenced by various factors, including the level of arousal and the elasticity of the skin. If your foreskin has retracted and is not returning to its original position, it could be due to temporary swelling or irritation from the prolonged activity. In some cases, this can lead to a condition known as phimosis, where the foreskin cannot be easily retracted. If this condition persists, it may be worth consulting a healthcare provider for evaluation.
5. Will exposing the glans lead to decreased sensitivity over time?
Exposing the glans can lead to changes in sensitivity. The glans is typically covered by the foreskin, which provides a certain level of protection and moisture. When exposed, it may become less sensitive over time due to increased friction and exposure to air. However, this is a gradual process, and many individuals adapt to changes in sensitivity. If you notice significant changes in your sexual function or sensitivity, it would be advisable to discuss this with a healthcare professional.
In summary, while some discomfort and changes can occur with excessive masturbation, many of these issues are temporary and resolve with time. However, if you experience persistent pain, changes in your anatomy, or any other concerning symptoms, it is crucial to consult a healthcare provider for a thorough evaluation. They can provide personalized advice and treatment options tailored to your specific situation. Remember, maintaining open communication with your healthcare provider about any sexual health concerns is essential for your overall well-being.
