Missed menstruation?
Hello doctor, I apologize for the inconvenience, but I would like to ask you a question.
My menstrual cycle has been around the 20th of the month for the last two months, but prior to that, it was around the 10th.
This month, it is already the 26th, and my period has not arrived yet.
However, for the past three days, I have noticed a small amount of blood when I urinate or have a bowel movement (sometimes red, sometimes brown).
I also have a bit of lower back pain, but the amount of blood is not like a normal period; it's just a little bit.
I had sexual intercourse this month, and we used a condom.
On the 24th, I bought a pregnancy test, and it showed negative.
I would like to ask the doctor, what is the likelihood of me being pregnant?
Xuan Xuan, 20~29 year old female. Ask Date: 2016/03/26

Hello, the chances of pregnancy are low, but it is best to take preventive measures beforehand.
If your period is more than a week late, it is advisable to see a gynecologist.
Wishing you good health.

It sounds like you are experiencing a lot of uncertainty and anxiety regarding your missed period and the symptoms you are experiencing. Let’s break down your concerns and provide some clarity on the situation.
Firstly, a missed period can be caused by various factors, including stress, hormonal imbalances, changes in weight, and of course, pregnancy. Given that your menstrual cycle has been regular in the past, the delay could be concerning, especially if you have had unprotected sexual intercourse. However, since you mentioned that you used protection, the likelihood of pregnancy is reduced, but not entirely eliminated. Condoms are generally effective, but they can fail due to breakage or slippage.
The symptoms you described—light spotting (which can be red or brown), mild lower back pain, and the absence of a full menstrual flow—can be indicative of several things. Spotting can occur for various reasons, including ovulation, hormonal fluctuations, or even early pregnancy. It’s important to note that some women experience light bleeding in early pregnancy, often referred to as implantation bleeding, which occurs when a fertilized egg attaches to the uterine lining. However, since your pregnancy test came back negative, this is less likely.
Given that you have taken a pregnancy test and it was negative, it’s possible that you are not pregnant. However, if your period does not arrive soon, or if your symptoms worsen, it would be advisable to consult with a healthcare provider. They may recommend a blood test, which is more sensitive than a urine test and can detect pregnancy earlier. Additionally, they can evaluate your symptoms more thoroughly to rule out other potential causes for your missed period and spotting.
Stress and anxiety can also play a significant role in menstrual irregularities. You mentioned feeling anxious, which can lead to hormonal changes that may affect your cycle. It’s essential to manage stress through relaxation techniques, exercise, or talking to someone about your feelings.
In terms of risks, if you are indeed pregnant, early pregnancy can come with its own set of concerns, including the risk of miscarriage, which is more common in the first trimester. However, most pregnancies progress without complications. If you are experiencing significant anxiety about the possibility of being pregnant or about your health in general, it may be beneficial to speak with a mental health professional. They can provide support and coping strategies to help you manage your feelings.
In summary, while the possibility of pregnancy exists, especially if there was any chance of condom failure, your negative pregnancy test and the nature of your symptoms suggest that it may not be the case. However, if your period does not arrive soon or if you continue to have symptoms, seeking medical advice is the best course of action. Remember, your health and peace of mind are important, and it’s always better to consult with a healthcare professional when in doubt.
