Desquamation issues?
Hello Doctor, I would like to ask about the situation regarding dryness and flaking of the glans.
It is more noticeable during erections, but there is no redness, pain, or itching.
I have followed your advice to apply baby oil after showering, but every morning after an erection when I need to urinate, I still notice some flakes on the glans.
I also consulted an online doctor who mentioned that it could be due to dry weather.
He suggested that if I feel it’s not moisturizing enough, I could switch to petroleum jelly.
I would like to ask you:
1.
Since petroleum jelly is a gel, should I keep the foreskin retracted after applying it?
2.
This issue has been bothering me since December of last year.
If applying petroleum jelly does not help, should I see a urologist or a dermatologist?
3.
Besides applying it after showering, can I also apply petroleum jelly in the morning?
4.
If this is due to dry weather, does that mean I will experience this condition every winter? I have not encountered this issue before, so I have more questions.
Thank you for your answers, Doctor.

Understanding dryness and flaking of the glans can be a common concern, especially in colder and drier climates. The symptoms you describe—dryness, flaking, and the presence of flakes on the glans during erections—can be attributed to several factors, including environmental conditions, hygiene practices, and skin sensitivity.
Causes of Dryness and Flaking
1. Environmental Factors: As you noted, moving to a colder and drier climate can significantly affect skin hydration. Dry air can lead to moisture loss from the skin, resulting in dryness and flaking. This is particularly true for sensitive areas like the glans, which may not be as resilient to environmental changes.
2. Hygiene Practices: While maintaining good hygiene is essential, over-washing or using harsh soaps can strip the skin of its natural oils, leading to dryness. It’s important to use mild, fragrance-free cleansers and to avoid excessive scrubbing.
3. Skin Conditions: Conditions such as eczema, psoriasis, or contact dermatitis can also cause dryness and flaking. If the skin appears red, inflamed, or itchy, it may indicate an underlying dermatological condition that requires further evaluation.
4. Irritation: Products like baby oil or petroleum jelly (Vaseline) can sometimes cause irritation, especially if they are not suitable for sensitive skin. It’s crucial to ensure that any product used is hypoallergenic and appropriate for the genital area.
Solutions and Recommendations
1. Use of Vaseline: Applying Vaseline after bathing can help lock in moisture. It’s generally recommended to apply it to clean, dry skin. You do not need to keep the foreskin retracted after applying it; simply apply a thin layer to the glans and surrounding area. If you feel discomfort or irritation, you may want to reconsider the product used.
2. Consulting a Specialist: If you find that using Vaseline does not alleviate the dryness and flaking, it would be wise to consult a healthcare professional. A dermatologist would be well-equipped to assess any potential skin conditions, while a urologist can address any concerns related to the genital area. Given that your symptoms have persisted for several months, seeking professional advice is advisable.
3. Frequency of Application: Vaseline can be applied as needed, not just after bathing. If you notice dryness throughout the day, you can reapply it to maintain moisture. However, ensure that the area is clean before reapplication to avoid trapping dirt or bacteria.
4. Seasonal Considerations: If dryness is primarily occurring in winter, it may be a seasonal issue. However, if you notice persistent symptoms regardless of the season, it may indicate a more chronic condition. Keeping the skin moisturized year-round is beneficial, especially in dry climates.
Additional Tips
- Hydration: Ensure you are drinking enough water, as hydration from within can also affect skin health.
- Humidifiers: Using a humidifier in your living space can help maintain moisture levels in the air, which may alleviate skin dryness.
- Avoid Irritants: Be cautious with any products applied to the genital area, including soaps, lotions, and lubricants. Opt for those specifically designed for sensitive skin.
In conclusion, while dryness and flaking of the glans can be exacerbated by environmental factors, proper care and attention to skin health can help manage the symptoms. If the condition persists or worsens, seeking medical advice is the best course of action to rule out any underlying issues.
