Burning sensation after ejaculation?

I have been in a relationship with my husband for over seven years, and before marriage, we practiced withdrawal without using condoms, and I did not get pregnant.
After marriage, we planned to conceive and have been having unprotected intercourse, but each time I experience a slight burning sensation in my uterus and noticeable menstrual cramps, which feel like mild contractions.
We have been trying to conceive for six months without success.
Could this be a case of semen allergy?

Naomi, 20~29 year old female. Ask Date: 2016/04/09

Dr. Cao Guotao reply Obstetrics and Gynecology

Hello, sometimes during excitement, there may be discomfort in the uterus or vagina, which is relatively normal.
Since you haven't successfully conceived for six months, it does not meet the definition of infertility, which is one year without pregnancy, so observation is sufficient.
Wishing you good health.

Experiencing a burning sensation after ejaculation can be concerning, especially when accompanied by other symptoms such as pelvic pain or difficulty conceiving. There are several potential causes for this sensation, and understanding them can help you and your partner address the issue effectively.

1. Semen Allergy: One possibility is a sensitivity or allergic reaction to proteins found in your partner's semen. This condition, known as human seminal plasma hypersensitivity, can lead to localized burning, itching, or swelling after intercourse. While rare, it is a condition that can cause discomfort and may affect fertility if it leads to avoidance of intercourse.

2. Infection: Another common cause of post-ejaculation burning is an infection, such as a urinary tract infection (UTI) or sexually transmitted infection (STI). These infections can cause inflammation and irritation in the genital area, leading to discomfort during and after ejaculation. Symptoms may include burning during urination, unusual discharge, or pelvic pain.

3. Prostatitis: Inflammation of the prostate gland, known as prostatitis, can also lead to burning sensations after ejaculation. This condition can be acute or chronic and may present with additional symptoms such as pelvic pain, difficulty urinating, or flu-like symptoms.

4. Pelvic Floor Dysfunction: Sometimes, pelvic floor muscle tension or dysfunction can cause pain during or after sexual activity. This can lead to a burning sensation and may be associated with other symptoms such as menstrual pain or discomfort during intercourse.

5. Hormonal or Structural Issues: Hormonal imbalances or structural issues within the reproductive system can also contribute to discomfort during intercourse and post-ejaculation burning. Conditions such as endometriosis or fibroids may cause significant pain and discomfort.

Given that you have been trying to conceive for six months without success, it is essential to consult with a healthcare provider. They can perform a thorough evaluation, including a physical examination and possibly laboratory tests, to determine the underlying cause of your symptoms.
In the meantime, consider the following steps:
- Track Symptoms: Keep a detailed record of your symptoms, including when they occur, their severity, and any other associated factors. This information can be helpful for your healthcare provider.

- Avoid Irritants: Consider avoiding potential irritants such as scented soaps, lubricants, or other products that may exacerbate your symptoms.

- Consult a Specialist: If your primary care provider is unable to identify the cause, consider seeing a specialist, such as a urologist or a reproductive endocrinologist, who can provide more targeted care.

- Discuss Fertility Options: If conception continues to be a challenge, discussing fertility options with your healthcare provider can help you explore potential treatments or interventions.

In conclusion, while a burning sensation after ejaculation can be distressing, it is essential to approach the situation with a comprehensive understanding of potential causes. Seeking medical advice is crucial to address your symptoms and any underlying issues that may be affecting your fertility.
