Left chest tightness with some pain upon pressure?

Hello doctor, I have angina, but recently I've been experiencing a runny nose, nasal congestion, and excessive phlegm.
I also have persistent tightness in my left chest, and pressing on a specific point in that area causes pain.
My throat feels tight, as if there's a foreign object in it.
I previously visited an ENT specialist who mentioned that I have throat inflammation and advised me to quit smoking.
However, I also have a history of gastric ulcers and gastroesophageal reflux disease (GERD).
I went to the emergency room before due to chest tightness and difficulty breathing, and the doctor suggested that I might be experiencing autonomic nervous system dysfunction.
Is this what autonomic nervous system dysfunction feels like? Can it cause chest tightness and neck tension?

Dr. Zhang Ziyan reply Pulmonology

Hello, it is recommended to quit smoking and undergo further outpatient examination and treatment, as there may be issues related to bronchitis or soft tissue inflammation.
Please refer to your treating physician for detailed information.

Understanding chest discomfort can be a complex issue, especially for patients with a history of cardiovascular conditions such as angina. Your symptoms of persistent chest tightness, localized pain upon pressing a specific point, and a sensation of tightness in the throat can be concerning, particularly given your medical history of angina, gastric ulcers, and gastroesophageal reflux disease (GERD).

Firstly, it’s important to recognize that chest discomfort can arise from various causes, which can be broadly categorized into cardiac and non-cardiac origins. Given your history of angina, any new or worsening chest symptoms should be evaluated promptly to rule out any potential cardiac issues. Angina itself is a type of chest pain caused by reduced blood flow to the heart muscle, and it can manifest as pressure, squeezing, or a feeling of fullness in the chest.
In your case, the combination of chest tightness, throat discomfort, and nasal symptoms could suggest several possibilities:
1. Gastroesophageal Reflux Disease (GERD): This condition can cause acid to flow back into the esophagus, leading to symptoms that mimic angina, including chest pain, throat tightness, and even respiratory symptoms like coughing or a sensation of mucus in the throat. The fact that you have a history of GERD makes this a likely contributor to your symptoms.

2. Musculoskeletal Issues: The localized pain upon pressing a specific point on your chest could indicate a musculoskeletal issue, such as costochondritis, which is inflammation of the cartilage connecting the ribs to the sternum. This condition can cause sharp pain that worsens with movement or pressure.

3. Anxiety and Autonomic Nervous System Dysregulation: The mention of autonomic nervous system dysregulation could point to anxiety or panic attacks, which can manifest as chest tightness, difficulty breathing, and a feeling of impending doom. Stress and anxiety can exacerbate physical symptoms, leading to a cycle of discomfort and worry.

4. Upper Respiratory Infections or Allergies: Your symptoms of nasal congestion and increased mucus production may indicate an upper respiratory infection or allergies, which can also contribute to feelings of chest tightness and discomfort.

Given these potential causes, it is crucial to approach your symptoms holistically. Here are some steps you can take:
- Consult with a Healthcare Provider: Given your history of angina and the new onset of symptoms, it is advisable to seek medical attention. A healthcare provider may recommend an electrocardiogram (ECG), stress testing, or imaging studies to evaluate your heart function and rule out any serious conditions.

- Manage GERD: If GERD is contributing to your symptoms, lifestyle modifications such as dietary changes, weight management, and medications like proton pump inhibitors (PPIs) may be beneficial.

- Address Anxiety: If anxiety is a significant factor, consider discussing this with your healthcare provider. Cognitive-behavioral therapy (CBT) and relaxation techniques can be effective in managing anxiety symptoms.

- Lifestyle Modifications: Quitting smoking, as advised by your ENT specialist, can significantly improve both respiratory and cardiovascular health. Additionally, regular physical activity, stress management techniques, and a balanced diet can help improve overall well-being.

In summary, while your symptoms may be multifactorial, it is essential to prioritize a thorough evaluation by a healthcare professional to ensure that any serious conditions are addressed. Understanding the interplay between your physical symptoms and emotional health can lead to a more comprehensive approach to your care. Remember, you are not alone in this, and seeking help is a vital step toward feeling better.
