Yellow and Thick Semen: Causes and Concerns - Urology

The semen is yellowish and viscous?

Hello, doctor.
A few weeks ago, after masturbating, I noticed some slight blood streaks in my semen.
After abstaining for a few days, I masturbated again and did not see any blood, but the semen was somewhat yellow and had a consistency similar to phlegm.
After abstaining for two weeks, when I masturbated again, the initial semen was more watery and transparent, but then it gradually became yellow and thicker, resembling lighter phlegm.
I also have a varicocele.
I would like to ask if there is a connection and whether this condition is normal or if I need to seek medical attention.

Dr. Li Xinyi reply Urology

The semen appears yellowish and viscous, which may be related to varicocele.
It is recommended to consult a urologist for a semen analysis and discuss the findings with the physician.

Understanding changes in semen color and consistency can be concerning, especially when accompanied by other symptoms. In your case, the presence of yellow, thick semen and occasional blood streaks raises several important points to consider.

Firstly, the color and consistency of semen can vary due to several factors, including hydration levels, diet, frequency of ejaculation, and underlying health conditions. Yellow semen can sometimes be attributed to the presence of substances such as urine, which can mix with semen, especially if there is a delay in ejaculation or if the bladder is not fully emptied before ejaculation. Additionally, certain foods (like asparagus) and supplements can also affect the color of semen.

The presence of blood in semen, known as hematospermia, can be alarming. While it can sometimes be benign and resolve on its own, it can also indicate underlying issues such as infections, inflammation, or vascular problems. Given that you have a history of varicocele (enlarged veins within the scrotum), this condition could potentially contribute to changes in semen quality. Varicoceles can affect sperm production and quality, leading to issues such as increased temperature in the testicular area, which may impact semen consistency and color.

The transition from yellow, thick semen to a more transparent and less viscous form after a period of abstinence suggests that your body is adjusting to changes in sexual activity and possibly hormonal levels. Semen typically becomes thicker with prolonged abstinence due to the accumulation of seminal fluid, which can lead to a more concentrated appearance.

However, if you continue to notice blood in your semen, or if you experience any pain, discomfort, or other unusual symptoms (such as changes in urination or sexual function), it is crucial to seek medical attention. A healthcare provider can perform a thorough evaluation, which may include a physical examination, ultrasound, or laboratory tests to rule out infections, tumors, or other serious conditions.

In summary, while some changes in semen color and consistency can be normal, especially in the context of your sexual activity and health history, the presence of blood and the specific characteristics of your semen warrant a professional evaluation. It is always better to err on the side of caution when it comes to reproductive health. Regular check-ups and open communication with your healthcare provider about any concerns you have will help ensure your well-being.
