Meniscus surgery
Hello doctor, I have been diagnosed with a meniscus tear.
I would like to ask if the current surgical options are limited to total meniscectomy, partial meniscectomy, and peripheral repair? Additionally, is it feasible with current medical technology in our country to use PRP therapy to repair the avascular zone of the meniscus, specifically the two-thirds area? Are there any successful cases?

Dr. Wang Jiacheng reply Orthopedics
The most direct and effective method is that the three types of PRP you described may not necessarily be effective.

Meniscus tears are a common knee injury, particularly among athletes and active individuals. When it comes to treatment options, you are correct that traditional surgical interventions include total meniscectomy (complete removal of the meniscus), partial meniscectomy (removal of only the damaged part), and meniscus repair, which aims to preserve as much of the meniscus as possible. The choice of procedure often depends on the type, location, and severity of the tear, as well as the patient's age, activity level, and overall knee health.
1. Surgical Options:
- Total Meniscectomy: This involves removing the entire meniscus. While it can relieve pain and restore function, it can lead to long-term complications, including osteoarthritis, due to the loss of the meniscus's cushioning and stabilizing functions.
- Partial Meniscectomy: This is a more conservative approach where only the damaged portion of the meniscus is removed. This option is often preferred as it preserves more of the meniscus, potentially reducing the risk of future joint problems.
- Meniscus Repair: This technique is ideal for tears located in the vascular (outer) region of the meniscus, where blood supply is better, allowing for healing. Repairing the meniscus can help maintain knee function and reduce the risk of arthritis.
2. PRP Treatment: Platelet-Rich Plasma (PRP) therapy is an emerging treatment option that utilizes the patient's own blood components to promote healing. PRP is rich in growth factors that can enhance tissue repair and regeneration. In the context of meniscus tears, PRP can be used in conjunction with surgical repair or as a standalone treatment for certain types of tears, particularly in the avascular (inner) region of the meniscus where blood supply is limited.
- Effectiveness: The success of PRP treatment can vary based on several factors, including the type of meniscus tear, the patient's age, and overall health. While there are promising studies indicating that PRP can improve outcomes in meniscus repair and reduce pain, more research is needed to establish standardized protocols and long-term effectiveness.
3. Current Medical Practices: In many countries, including the United States, PRP therapy is becoming more widely accepted in orthopedic practices. However, its use for meniscus tears, especially in the avascular zone, is still considered experimental by some practitioners. It is essential to consult with an orthopedic surgeon who specializes in knee injuries to discuss the best treatment options tailored to your specific situation.
4. Success Stories: There have been successful cases of PRP treatment in conjunction with meniscus repair, particularly in athletes looking to return to high levels of activity. However, the outcomes can be variable, and not all patients experience the same level of improvement. Factors such as the patient's adherence to rehabilitation protocols and the specific characteristics of the tear play significant roles in recovery.
In conclusion, while traditional surgical options for meniscus tears remain the standard, PRP therapy offers an exciting avenue for enhancing recovery and potentially improving outcomes. It is crucial to have a thorough discussion with your orthopedic surgeon about the risks and benefits of each treatment option, including PRP, to make an informed decision that aligns with your health goals and lifestyle.
