Irregular menstruation
Hello! Eight weeks after giving birth, I experienced my first menstrual cycle, which was very heavy.
Within 2-3 hours, the entire pad was almost completely soaked, and there were large blood clots.
This lasted for several days (5-7 days).
I consulted a Western medicine doctor, who said it was irregular menstruation and not a true miscarriage.
After receiving an injection and medication, it stopped the next day.
The doctor mentioned that after taking the second course of medication (three days, three times a day), a normal menstrual cycle would follow.
After finishing the medication, my menstrual cycle returned the next day, but it was the same as before, lasting for four days with heavy flow, requiring a pad change every 2-3 hours, and this time accompanied by lower abdominal pain.
This is my second child; I had similar issues during my first pregnancy.
I would like to ask:
1.
What is meant by irregular menstruation?
2.
In Traditional Chinese Medicine, are there any prescriptions for treatment?
3.
What could be the cause? (Could it be due to improper postpartum care?)
4.
How should I manage my health in the future, and what dietary considerations should I keep in mind?
Anqi, 30~39 year old female. Ask Date: 2002/09/03
Dr. Chen Weili reply Traditional Chinese Medicine
Hello, Anqi:
1.
Generally speaking, any irregularity in menstruation, such as a missed period or abnormal flow, can be classified as menstrual irregularity.
Based on your description, it seems to be caused by endocrine disorders leading to excessive endometrial hyperplasia, which results in shedding and bleeding when it becomes unmanageable.
2.
From a Traditional Chinese Medicine perspective, you might consider using a modified version of the Xiong Gui Jiao Ai Decoction for treatment, but it is best to consult a licensed TCM practitioner for a comprehensive assessment before proceeding.
3.
Since you experienced this issue during your first pregnancy, it may be related to your individual constitution.
Additionally, if you did not adequately address your deficiencies during the postpartum period, this could contribute to the situation.
However, this is quite common among postpartum women.
4.
For your future well-being, seeking assistance from a TCM practitioner for proper regulation would be a wise approach.

More Info
After childbirth, many women experience irregular menstruation, which can be concerning and confusing. Your situation, characterized by heavy bleeding, large blood clots, and abdominal pain, is not uncommon, especially in the postpartum period. Let's address your questions one by one.
1. What is irregular menstruation?
Irregular menstruation, or "dysmenorrhea," refers to menstrual cycles that are inconsistent in timing, flow, or duration. After childbirth, it is common for women to experience changes in their menstrual cycle due to hormonal fluctuations, physical changes in the body, and the stress of caring for a newborn. The first few cycles can be particularly erratic as the body readjusts to its pre-pregnancy state. Heavy bleeding, known as menorrhagia, can occur as the uterus sheds its lining, especially if there are retained products of conception or hormonal imbalances.
2. Are there traditional Chinese medicine (TCM) remedies available?
Traditional Chinese medicine offers various herbal remedies and acupuncture techniques that may help regulate menstrual cycles and alleviate symptoms. Common TCM approaches include using herbs that promote blood circulation and reduce stagnation, such as Dang Gui (Angelica sinensis) and Chuan Xiong (Ligusticum chuanxiong). However, it is essential to consult with a qualified TCM practitioner who can assess your specific condition and recommend an appropriate treatment plan tailored to your needs.
3. What causes irregular menstruation after childbirth?
Several factors can contribute to irregular menstruation postpartum:
- Hormonal Changes: After childbirth, hormone levels, particularly estrogen and progesterone, fluctuate significantly. This can lead to irregular ovulation and menstrual cycles.
- Uterine Recovery: The uterus needs time to return to its pre-pregnancy size and condition. If there are any retained placental fragments, this can cause prolonged bleeding.
- Physical and Emotional Stress: The demands of caring for a newborn can lead to stress, which may affect menstrual regularity.
- Diet and Lifestyle: Poor nutrition, lack of sleep, and inadequate postpartum care can also contribute to menstrual irregularities.
4. How to regulate menstruation and dietary considerations?
To help regulate your menstrual cycle and improve overall health postpartum, consider the following:
- Balanced Diet: Focus on a nutrient-rich diet that includes plenty of fruits, vegetables, whole grains, lean proteins, and healthy fats. Foods rich in iron (like leafy greens, beans, and lean meats) can help replenish blood loss.
- Hydration: Drink plenty of water to stay hydrated, which is crucial for overall health and recovery.
- Rest and Recovery: Prioritize rest and recovery during the postpartum period. Sleep deprivation can exacerbate hormonal imbalances.
- Gentle Exercise: Engage in light physical activity, such as walking or postpartum yoga, to promote circulation and overall well-being.
- Consult Healthcare Providers: Regular check-ups with your healthcare provider are essential. If heavy bleeding persists or worsens, further evaluation may be necessary to rule out any underlying conditions.
In summary, irregular menstruation after childbirth is common and can be influenced by various factors, including hormonal changes, physical recovery, and lifestyle. While TCM may offer supportive remedies, it is crucial to work closely with your healthcare providers to ensure that any underlying issues are addressed and that you receive appropriate care. If your symptoms persist or worsen, do not hesitate to seek medical attention for further evaluation and management.
