Timing of Fish Oil Intake
Hello, Doctor.
I have two questions I would like to ask you.
Thank you!
1.
I am currently taking Azathioprine 50mg (one in the morning) for atopic dermatitis, along with antihistamines and MethyIPREDNISolone 4mg (one in the morning and one in the evening).
If I want to take fish oil, should I take it before my meals or after taking my medications?
2.
The doctor mentioned that Azathioprine 50mg is prescribed to help reduce the side effects of corticosteroids.
Could you please tell me what adverse effects might occur after stopping this medication? Will it have a rebound effect similar to tapering off corticosteroids?
Mr. Zhang, 20~29 year old female. Ask Date: 2016/05/30
Dr. Liao Lingmei reply Pharmacology
Hello:
1.
There are concerns regarding the interaction of the active ingredient in fish oil, Omega-3-Acid Ethyl Esters, with anticoagulant or antiplatelet medications.
However, there is no documented interaction with the medications you are currently taking.
You may take it as per your personal medication habits or as recommended by the manufacturer, in whole form.
2.
Atopic dermatitis may require the use of Azathioprine in conjunction with corticosteroids, as assessed by a physician, primarily to achieve immunosuppressive effects.
The decision regarding dosage and timing of discontinuation must be based on professional judgment, weighing the benefits against the potential side effects.
The occurrence of side effects upon discontinuation can vary from person to person.
However, if any discomfort arises, it is usually temporary, so there is no need to worry.
Please ensure to complete the appropriate treatment in collaboration with your physician.
Wishing you health and happiness!

When considering the optimal timing for fish oil supplementation alongside medications such as Azathioprine and MethyIPREDNISolone, it's essential to understand both the pharmacological interactions and the physiological effects of these substances.
1. Timing of Fish Oil Supplementation: Fish oil, which is rich in omega-3 fatty acids (EPA and DHA), can be taken at various times relative to your medications. Generally, it is advisable to take fish oil after meals to enhance absorption and minimize gastrointestinal discomfort. Since you are currently taking Azathioprine and MethyIPREDNISolone, it would be prudent to take fish oil after your meals and after you have taken your medications. This approach ensures that the absorption of the medications is not impaired by the presence of fish oil, as high-fat meals can sometimes affect the absorption of certain drugs. However, there is no direct interaction reported between fish oil and Azathioprine or MethyIPREDNISolone, so you can choose a timing that fits your routine best, as long as it is after meals.
2. Discontinuation of Azathioprine: Azathioprine is an immunosuppressive medication often used to manage conditions like atopic dermatitis, particularly when corticosteroids are being tapered down to reduce their side effects. When discontinuing Azathioprine, it is crucial to do so under the guidance of your healthcare provider. Stopping Azathioprine suddenly may lead to a resurgence of the underlying condition, as the immune system may rebound after suppression. This rebound effect can sometimes be similar to the withdrawal symptoms seen with corticosteroids, where the body reacts to the sudden absence of the medication.
The side effects of stopping Azathioprine can vary from person to person. Some individuals may experience a return of their symptoms, while others may not notice significant changes. It is essential to monitor for any signs of flare-ups or adverse reactions during the tapering process. Your healthcare provider may suggest a gradual reduction in dosage rather than an abrupt stop to minimize potential withdrawal effects and to monitor your response closely.
In summary, for optimal timing, take fish oil after meals and after your medications. When it comes to discontinuing Azathioprine, do so under medical supervision to manage any potential side effects effectively. Always communicate with your healthcare provider regarding any concerns or changes in your condition during this process.
