Can I Take Cephalexin After Using Mycomb Cream? - Pharmacology

I am currently taking cephalexin, but I have previously used Mycomb?

I would like to ask if it's safe for me to take cephalexin 500mg, which my clinic physician prescribed for me to take four times a day.
Last week, I had a different hospital apply Meikang ointment in my ear, and I heard that cephalexin and similar antibiotics can enhance the ototoxicity of the antibiotics in Meikang ointment.
Can I take cephalexin?

Yao Shi Ni Hao, 20~29 year old female. Ask Date: 2016/07/21

Dr. Hou Fangmin reply Pharmacology

Hello:
Meikang Ointment contains Nystatin ……………………………… 100,000 units
Neomycin (as sulfate)……………… 2.5 mg
Gramicidin……………………………… 0.25 mg
Triamcinolone acetonide……………… 1.0 mg
This ointment has four different pharmacological effects: anti-inflammatory, antipruritic, antibacterial, and antifungal.

(1) Nystatin: A unique antifungal agent effective against skin infections caused by Candida albicans and other fungal organisms.

(2) The combination of Neomycin and Gramicidin provides a broad-spectrum antibacterial effect and exhibits strong bactericidal activity against antibiotic-resistant bacteria.

(3) Triamcinolone Acetonide: A potent corticosteroid with anti-inflammatory, antipruritic, and anti-allergic properties, it acts quickly and provides complete and prolonged relief for inflammatory skin conditions.

Indications: Eczema, allergic and inflammatory skin diseases, secondary bacterial or fungal infections (Candida infections).

Cephalexin: It has a broad-spectrum bactericidal effect against common clinical Gram-positive and Gram-negative bacteria.
It is rapidly absorbed after oral administration, reaching peak blood concentration in a short time, and is excreted in active form via the kidneys in urine, making it suitable for the treatment of various infections.

The antibiotic you are taking is a standalone antibiotic, while the ointment you are applying contains antifungal, corticosteroid, and antibiotic components.
There will be no interaction between the two, so taking cephalexin is not a problem.

Wishing you peace and joy.

When considering the use of Cephalexin after applying Mycomb Cream, it's essential to understand the components of both medications and their potential interactions. Mycomb Cream contains a combination of antifungal and antibiotic agents, including Neomycin and Gramicidin, as well as Triamcinolone, a corticosteroid. Cephalexin is a cephalosporin antibiotic that is commonly used to treat bacterial infections.

The concern you mentioned regarding the potential for increased ototoxicity (ear toxicity) when using Cephalexin in conjunction with Mycomb Cream is valid. Neomycin, one of the components of Mycomb Cream, is known to have ototoxic effects, particularly when used in high doses or for prolonged periods. While Cephalexin itself is not typically associated with ototoxicity, the combination of these medications could theoretically increase the risk of ear-related side effects, especially if there is an underlying condition affecting the ears.

However, it is crucial to note that the risk of ototoxicity primarily arises from the use of Neomycin, especially when it is applied to sensitive areas or in cases of pre-existing ear conditions. If you have already applied Mycomb Cream to your ear and are now prescribed Cephalexin, it is advisable to monitor for any unusual symptoms such as hearing changes, tinnitus (ringing in the ears), or dizziness.

In general, if your healthcare provider has prescribed Cephalexin, they have likely assessed that the benefits of treating your infection outweigh the potential risks. It is always best to follow your doctor's advice and discuss any concerns you have about medication interactions or side effects with them directly. If you experience any adverse effects or have concerns about the combination of these medications, do not hesitate to reach out to your healthcare provider for further guidance.

In summary, while there is a theoretical risk of increased ototoxicity when using Cephalexin after Mycomb Cream, the actual risk may be low, especially if used as directed. Always prioritize communication with your healthcare provider regarding any medications you are taking, and report any concerning symptoms promptly.
