Pain below the right rib cage?
I had a cough for half a month, and the day I stopped coughing, I started experiencing pain under my right rib cage.
When I lie down and feel the urge to cough or sneeze, I have to press on the right side of my body to avoid the pain; otherwise, I have to crouch down to alleviate the pain from sneezing or coughing.
Sometimes, if I don't manage it well, it leads to severe pain that seems to radiate to the right side of my chest.
Generally, when standing or sitting, I don't feel much pain unless I press on the tender spot.
However, when I lie flat, I feel tightness and significant pain in the area under my right rib cage...
even light touching is very painful.
What could be the cause of this?

Dr. Zhou Ziguang reply Pulmonology
Hello: Pain upon palpation is more likely to be due to rib or muscle pain.
You may consider getting a chest X-ray for further evaluation.
Thank you!

More Info
Understanding pain under the right rib can be complex, as it may stem from various underlying causes. Based on your description, it seems that the pain began after a prolonged cough, which could suggest a few potential issues. Here, I will outline some possible causes and solutions for your pain, while also providing additional context based on medical knowledge.
Possible Causes of Pain Under the Right Rib
1. Muscle Strain: Coughing for an extended period can lead to muscle strain in the intercostal muscles, which are located between the ribs. This strain can cause localized pain that may worsen with certain movements, such as coughing, sneezing, or even lying down.
2. Costochondritis: This condition involves inflammation of the cartilage that connects a rib to the sternum. It can cause sharp pain in the rib area, particularly when moving or pressing on the affected area. The pain may mimic that of a heart attack or other serious conditions, but it is usually benign.
3. Rib Fracture: Although you mentioned that you did not experience any trauma, repetitive coughing can sometimes lead to stress fractures in the ribs, especially in individuals with weakened bones. This could explain the localized pain and sensitivity when pressure is applied.
4. Pleural Irritation: If the cough was due to a respiratory infection, it might have caused irritation of the pleura (the lining around the lungs), leading to pain that can be felt in the rib area. This pain often worsens with deep breaths, coughing, or sneezing.
5. Gallbladder Issues: Although less likely given the context, pain under the right rib can sometimes be related to gallbladder problems, such as gallstones or cholecystitis. This pain often radiates to the right shoulder or back and may be accompanied by digestive symptoms.
6. Referred Pain: Pain in the rib area can sometimes be referred from other organs, such as the liver or kidneys. Conditions affecting these organs can manifest as rib pain, particularly if there is inflammation or infection.
Solutions and Recommendations
1. Rest and Ice: If the pain is due to muscle strain, resting the affected area and applying ice can help reduce inflammation and alleviate pain. Ice should be applied for 15-20 minutes every few hours.
2. Pain Relief Medications: Over-the-counter nonsteroidal anti-inflammatory drugs (NSAIDs), such as ibuprofen or naproxen, can help manage pain and reduce inflammation. However, consult with a healthcare provider before starting any medication, especially if you have underlying health conditions.
3. Gentle Stretching and Breathing Exercises: Engaging in gentle stretching and deep breathing exercises can help relieve tension in the chest and rib area. However, avoid any movements that exacerbate the pain.
4. Physical Therapy: If the pain persists, consider consulting a physical therapist who can provide tailored exercises and treatments to strengthen the muscles around the rib cage and improve flexibility.
5. Medical Evaluation: If the pain continues or worsens, it is crucial to seek a medical evaluation. A healthcare provider may recommend imaging studies, such as an X-ray or ultrasound, to rule out fractures or other underlying conditions.
6. Monitor Symptoms: Keep track of any additional symptoms, such as fever, difficulty breathing, or changes in appetite, as these may indicate a more serious condition that requires immediate medical attention.
Conclusion
Pain under the right rib can arise from various causes, particularly following a period of intense coughing. While muscle strain and costochondritis are common culprits, it is essential to monitor your symptoms and consult with a healthcare professional if the pain persists or worsens. Early intervention can lead to better outcomes and help you return to your normal activities.
