MC and medication abortion issues?

I would like to inquire about my situation.
I underwent a medical abortion on July 18.
During a follow-up appointment, the doctor informed me that everything was cleared out, with just a small amount of membrane remaining.
However, after taking emergency contraception on July 25 and August 9, I still experienced a little bit of brown discharge until August 11.
Yet, on August 13, I started to have a flow similar to my menstrual cycle.
Does this count as an early period?

Chung, 20~29 year old female. Ask Date: 2016/08/14

Dr. Cao Guotao reply Obstetrics and Gynecology

Hello, emergency contraceptive pills can sometimes cause irregular menstruation.
If you started experiencing bleeding similar to your menstrual cycle on August 13, it should be fine to monitor it.
It's best to use preventive measures beforehand.
Wishing you good health.

After a medication abortion, it is not uncommon for women to experience irregular bleeding patterns, including early menstruation. In your case, it appears that you had a medication abortion on July 18, and subsequent bleeding and spotting occurred, which can be attributed to the hormonal changes and the body’s adjustment following the procedure.

Understanding Early Menstruation After Medication Abortion
1. Hormonal Fluctuations: After a medication abortion, the levels of hormones such as estrogen and progesterone drop significantly. This hormonal shift can lead to irregular bleeding patterns, including spotting or light bleeding that may not resemble a typical menstrual period. The presence of brown discharge is often old blood that is being expelled from the uterus, which can happen as the lining sheds.

2. Timing of Menstrual Cycle: The menstrual cycle can be affected by various factors, including stress, hormonal changes, and the body’s recovery from the abortion. It is possible for your menstrual cycle to resume earlier than expected after an abortion, especially if you have taken emergency contraception (the "morning-after pill") after the procedure. These pills contain hormones that can alter your menstrual cycle temporarily.

3. Your Specific Case: In your situation, you mentioned experiencing brown spotting until August 11, followed by what seems to be a heavier flow starting on August 13. This could indeed be considered an early menstruation, especially since the typical menstrual cycle can range from 21 to 35 days. If your cycle was regular before the abortion, and you are now experiencing bleeding within a shorter time frame than usual, it could be classified as early menstruation.

Factors to Consider
- Cycle Regularity: If your menstrual cycles were regular prior to the abortion, the early onset of bleeding may indicate that your body is adjusting to the hormonal changes. If your cycles were irregular before, this could also contribute to the unpredictability of your bleeding patterns.

- Emergency Contraception: Taking emergency contraception can lead to changes in your menstrual cycle. It is designed to prevent pregnancy after unprotected intercourse, but it can also cause changes in the timing and flow of your next period.

- Follow-Up Care: It is essential to maintain communication with your healthcare provider regarding any unusual bleeding patterns or concerns. If you experience heavy bleeding (soaking through a pad or tampon in an hour), severe pain, or any signs of infection (fever, chills, foul-smelling discharge), you should seek medical attention promptly.

Conclusion
In summary, experiencing early menstruation after a medication abortion is not uncommon and can be attributed to hormonal changes and the effects of any subsequent medications, such as emergency contraception. It is important to monitor your symptoms and maintain open communication with your healthcare provider to ensure that your recovery is progressing normally. If you have any further concerns or if your bleeding patterns do not stabilize, do not hesitate to reach out to your healthcare provider for guidance and support.
