Post-banding of internal hemorrhoids

Hello Doctor, I underwent vaginal surgery on August 5th.
For the first couple of days, I felt a strong urge to have a bowel movement, but on August 7th, the sensation was still intense.
I tried to have a bowel movement, but I struggled for a long time and couldn't go, which resulted in significant pain in my anus...
It hurt so much that even slight straining was excruciating.
On August 8th, the pain persisted, and I wondered if it could be related to hemorrhoids.
After seeing a doctor, I was informed that my internal hemorrhoid had prolapsed, and the doctor performed a rubber band ligation.
The postoperative pain intensified; whether standing, sitting, or lying down, the pain was unbearable.
I had to rely on painkillers daily just to feel somewhat comfortable, especially since standing would trigger a very strong urge to have a bowel movement, causing my legs to feel weak after about five minutes.
After having a bowel movement, the pain was even worse! On the morning of August 15th, I saw the doctor again, who mentioned that the rubber band had fallen off and that my condition had improved somewhat.
However, a small external hemorrhoid that previously caused me no issues had grown larger...
The doctor simply advised me to take painkillers if the pain continued.
Oh my...
I really want to understand why I am experiencing such pain, to the point where it feels like my entire backside is hurting...
Furthermore, during my recent visits, the doctor’s examinations of my anus have been extremely painful...
Because of this, I am hesitant to apply the ointment internally and only dare to apply it on the surface.
Warm sitz baths feel comfortable, but the pain returns once I stand up...
Is it possible that the higher temperature of the water is affecting me? Can the doctor help me with these questions?

Answer: It is unclear what type of vaginal surgery was performed on August 5th, but it is possible that your current issues are related to that surgery.
If it was a vaginal reconstructive surgery, sometimes the sutures may be placed too deeply, potentially involving the rectum, which could lead to your current symptoms.
If it is simply a hemorrhoid issue, medication treatment combined with warm sitz baths can usually alleviate the symptoms.
However, if you are experiencing severe pain, it is best to consult a physician for an examination.

Post-surgery pain after hemorrhoid ligation, particularly after a procedure like rubber band ligation, can be quite significant and distressing for patients. It is essential to understand the reasons behind this pain and how to manage it effectively.

Firstly, the pain you are experiencing is not uncommon after hemorrhoid surgery. The procedure itself involves placing a rubber band around the base of the hemorrhoid, which cuts off its blood supply. This causes the hemorrhoid to shrink and eventually fall off. However, the process can lead to inflammation and irritation in the surrounding tissues, resulting in pain. This pain can be exacerbated by bowel movements, as straining can put additional pressure on the surgical site.

The sensation of needing to have a bowel movement, even when you may not need to, is often due to the irritation of the anal canal and rectum following surgery. This can lead to a cycle of discomfort where the fear of pain during bowel movements causes anxiety, which in turn can lead to more straining and discomfort.

In your case, it seems that the pain has been quite severe, especially when attempting to have a bowel movement. This could be due to a few factors:
1. Inflammation and Swelling: After surgery, the tissues around the surgical site can become inflamed, leading to increased sensitivity and pain.

2. Straining: If you are having difficulty passing stool, straining can exacerbate pain. It is crucial to maintain soft stools to minimize straining.

3. Potential Complications: In some cases, complications such as thrombosed external hemorrhoids or infections can occur, leading to increased pain. If your external hemorrhoid has enlarged, it may be contributing to your discomfort.

4. Nerve Sensitivity: After surgery, the nerves in the area may become more sensitive, leading to heightened pain responses.

To manage your pain and discomfort, consider the following strategies:
- Pain Management: Continue to use prescribed pain medications as directed. Over-the-counter pain relievers like ibuprofen or acetaminophen can also help, but consult your doctor before taking any new medications.

- Stool Softeners: To prevent straining during bowel movements, consider using stool softeners or a fiber supplement. Staying hydrated and consuming a diet rich in fiber can also help maintain softer stools.

- Warm Sitz Baths: Taking warm sitz baths can provide relief from pain and help soothe the area. Ensure the water is warm but not too hot, as excessive heat can lead to further irritation.

- Avoiding Irritants: Be cautious with topical treatments. If applying ointments or creams is painful, it may be best to avoid inserting anything into the anal canal until the area has healed sufficiently.

- Follow-Up Care: Regular follow-ups with your healthcare provider are essential. If your pain persists or worsens, or if you notice any signs of infection (such as increased redness, swelling, or discharge), you should seek medical attention promptly.

In summary, while post-surgery pain after hemorrhoid ligation can be intense, understanding the causes and implementing effective pain management strategies can significantly improve your comfort. If your symptoms do not improve or if you have concerns about your recovery, do not hesitate to reach out to your healthcare provider for further evaluation and guidance.
