Pain may occur after hemorrhoid ligation surgery for internal hemorrhoids?
Dear Doctor: During my last vasectomy, I experienced no pain at all.
However, after this recent procedure, I was in so much pain on the first and second days that I couldn't sleep.
The doctor prescribed pain medication, but my condition hasn't improved.
I feel pain when walking and sitting.
What could be the cause? I've seen others with similar situations; could it be due to irritation of an external hemorrhoid? Now on the fourth day, the pain has improved significantly, and I can walk and sit without discomfort, but I still experience pain when using the restroom.
How long will this pain last? Would sitting in warm water help improve the situation? I would appreciate your guidance.
Thank you.

Typically, hemorrhoid ligation is performed for internal hemorrhoids; however, in some patients with mixed internal and external hemorrhoids, it is possible that external hemorrhoids may also be ligated for therapeutic purposes.
In your case, since the pain was most severe on the first and second days and then gradually alleviated, it resembles a complication following the ligation procedure.
Please schedule an earlier follow-up appointment to confirm your condition.
Thank you.

More Info
Post-surgery pain after hemorrhoid ligation can be a common experience, and it can vary significantly from patient to patient. In your case, it seems that you experienced considerable discomfort in the first few days following the procedure, which is not unusual. Let's break down the potential causes of your pain and discuss recovery tips.
Causes of Pain After Hemorrhoid Ligation
1. Surgical Trauma: The ligation procedure involves placing a rubber band around the base of the hemorrhoid to cut off its blood supply. This process can cause trauma to the surrounding tissues, leading to inflammation and pain. The first few days post-surgery are often when the pain is most intense as the body begins to heal.
2. Type of Hemorrhoid: If the ligation was performed on an external hemorrhoid, it may lead to more pain compared to internal hemorrhoids. External hemorrhoids are more sensitive and can cause significant discomfort during the healing process.
3. Infection: Although rare, there is a possibility of infection following any surgical procedure. Signs of infection include increased redness, swelling, warmth, and discharge from the surgical site. If you notice any of these symptoms, it’s essential to contact your healthcare provider.
4. Constipation and Straining: Post-surgery, many patients experience constipation due to pain medications or changes in diet. Straining during bowel movements can exacerbate pain, especially if the ligation site is still healing.
Recovery Tips
1. Pain Management: Continue to take the prescribed pain medications as directed. If the pain persists despite medication, consult your doctor for alternatives or adjustments in your pain management plan.
2. Warm Sitz Baths: Taking warm sitz baths several times a day can help soothe the area, reduce inflammation, and promote healing. This practice can also ease pain during bowel movements.
3. Dietary Adjustments: To prevent constipation, increase your fiber intake by consuming fruits, vegetables, and whole grains. Staying hydrated is also crucial, so drink plenty of water.
4. Avoid Straining: When using the restroom, try to relax and avoid straining. If necessary, consider using a stool softener to make bowel movements easier.
5. Rest and Recovery: Allow your body time to heal. Avoid heavy lifting or strenuous activities that could aggravate the surgical site.
6. Follow-Up Care: Keep your follow-up appointments with your healthcare provider to monitor your recovery. If your pain worsens or you experience new symptoms, don’t hesitate to reach out for medical advice.
Duration of Pain
The duration of pain after hemorrhoid ligation can vary. Many patients report significant improvement within a few days to a week. However, some discomfort, especially during bowel movements, may persist for a couple of weeks as the area heals. It’s essential to be patient and follow the recovery tips mentioned above.
In summary, while your experience of pain after the ligation procedure is concerning, it is not uncommon. By managing your pain effectively, maintaining a healthy diet, and practicing good hygiene, you can facilitate your recovery. If you have any ongoing concerns or if your symptoms do not improve, please consult your healthcare provider for further evaluation and management.
