Missed menstruation?

Hello Doctor: I had unprotected intercourse with ejaculation on March 5th when my period just started.
After that, my periods occurred at the beginning of April, from April 22 to 26, from May 21 to 25, and from June 13 to 17.
Now, it has been two months without a period.
Is it possible that I am pregnant, or could this just be a case of hormonal imbalance? During this time, I have also experienced weight gain and acne around my chin and mouth.
Thank you.

Amanda, 20~29 year old female. Ask Date: 2016/08/28

Dr. Huang Yiwen reply Obstetrics and Gynecology

Hello Ms.
Amanda: Your period has not come for two months, which could indicate menstrual irregularity, but it may also be a sign of pregnancy.
I recommend taking a urine test for a more accurate assessment.
The weight gain and acne you are experiencing could be due to irregular sleep patterns and poor diet, which may have contributed to the menstrual irregularity as well.

When considering the possibility of pregnancy versus hormonal imbalance in the context of missed periods, it is essential to evaluate several factors, including the timing of sexual activity, the regularity of menstrual cycles, and accompanying symptoms.

Based on your timeline, you had unprotected intercourse on March 5, and your menstrual cycles continued normally through April, May, and June. However, you have now missed your period for two months. This raises the question of whether you could be pregnant or if there is an underlying hormonal issue.

1. Pregnancy Possibility: Given that you had unprotected intercourse, there is always a possibility of pregnancy. However, since you experienced normal menstrual cycles after the date of intercourse, it is less likely that you are pregnant from that encounter. Typically, if conception occurs, you would expect to miss your period within the following month. That said, it is still advisable to take a home pregnancy test to rule out pregnancy, especially since you have missed two periods.

2. Hormonal Imbalance: The symptoms you describe—weight gain and acne—can be indicative of hormonal changes. Conditions such as polycystic ovary syndrome (PCOS) can lead to irregular menstrual cycles, weight gain, and acne due to elevated androgen levels. Stress, significant weight changes, and other lifestyle factors can also contribute to hormonal imbalances that affect your menstrual cycle.

3. Other Factors: Consider other potential causes for missed periods, such as thyroid disorders, excessive exercise, or significant changes in diet. These factors can disrupt the hypothalamic-pituitary-ovarian axis, leading to irregularities in your menstrual cycle.

4. Next Steps: It is crucial to consult with a healthcare provider for a thorough evaluation. They may recommend blood tests to check hormone levels, including pregnancy tests, thyroid function tests, and possibly tests for other hormonal conditions. A pelvic ultrasound may also be considered to assess the ovaries and uterus for any abnormalities.

5. Lifestyle Considerations: In the meantime, maintaining a healthy lifestyle can help regulate your hormones. This includes a balanced diet, regular exercise, and managing stress levels. If you suspect that weight gain is related to hormonal changes, discussing dietary adjustments with a nutritionist may also be beneficial.

In conclusion, while there is a possibility of pregnancy, the more likely scenario given your recent menstrual history and symptoms may be a hormonal imbalance. Taking a pregnancy test and consulting with a healthcare provider will provide clarity and help you determine the appropriate next steps for your health.
