How to get pregnant?
Dr.
Tsao: Hello! I am 39 years old (I discovered I had uterine fibroids about 5 to 6 years ago, plus I have been staying up late for a long time!) My husband is 59 years old, and we have been married for less than 4 months! However, I really want to get pregnant! The problem is that since around September, my menstrual cycle has been irregular! I got my period on September 25, then again on October 25, and then it came on November 4.
So, I visited a gynecologist on November 8.
The doctor said I have insufficient progesterone, which is causing irregular ovulation.
They prescribed me progesterone medication, and then my period came again on November 15! After discussing my desire to get pregnant with the doctor, they provided me with a basal body temperature chart to track my temperature daily.
I found a lot of information stating that a sustained high temperature for more than 10 or 15 days could indicate a chance of pregnancy! However, since I started measuring on November 9, the longest continuous high temperature I recorded was only 8 days in December! Additionally, I started traditional Chinese medicine treatment around November, but I have not seen any results yet! The lowest temperature in January was 36.44 on January 2 and 36.47 on January 6.
The high temperatures were 36.84 on January 7, 36.96 on January 8, 36.92 on January 9, and 36.91 on January 10.
I visited another gynecologist on January 8, and the doctor told me to start taking progesterone medication from January 12 (Thursday).
However, I forgot when I should start having intercourse.
Is it from today, January 11 (Wednesday), or January 12 (Thursday)? Also, what are my chances of getting pregnant? Thank you!

Hello, you should be in the high-temperature phase, indicating that ovulation has occurred.
It is advisable to observe and let things take their natural course.
If you still have concerns, you may consult your regular obstetrician-gynecologist.
Wishing you good health.

Understanding fertility, especially at the age of 39 with irregular cycles, can be quite challenging. Given your situation, there are several factors to consider that can help improve your chances of conceiving.
Firstly, it's important to recognize that age plays a significant role in fertility. At 39, while you are not yet classified as a high-risk pregnancy, fertility does tend to decline as women age. The quality and quantity of eggs decrease, which can lead to irregular menstrual cycles and difficulties in conceiving. Your history of uterine fibroids and lifestyle factors, such as long nights and stress, can also contribute to hormonal imbalances that affect ovulation.
Your doctor has indicated that you have insufficient progesterone, which is crucial for maintaining a healthy pregnancy. Progesterone helps prepare the uterine lining for implantation of a fertilized egg. If your cycles are irregular, it can be difficult to predict ovulation, making it harder to time intercourse for conception. The use of progesterone supplements, as prescribed by your doctor, can help regulate your cycles and support ovulation.
Regarding your question about when to have intercourse, the timing is crucial. Typically, ovulation occurs about 14 days before your next expected period. If you are taking progesterone starting on January 12, it is likely that your doctor wants you to have intercourse in the days leading up to and including ovulation. Since you are tracking your basal body temperature (BBT), you should look for a sustained rise in temperature, which indicates that ovulation has occurred. Ideally, you should aim to have intercourse in the days leading up to this temperature spike and for a few days afterward to maximize your chances of conception.
In terms of improving your fertility, here are some additional tips:
1. Healthy Lifestyle: Focus on a balanced diet rich in fruits, vegetables, whole grains, and lean proteins. Maintaining a healthy weight can also improve your chances of conceiving.
2. Regular Exercise: Moderate exercise can help regulate your menstrual cycle and improve overall health. However, avoid excessive exercise, as it can negatively impact fertility.
3. Stress Management: High stress levels can affect hormonal balance and ovulation. Consider incorporating stress-reducing activities such as yoga, meditation, or deep-breathing exercises into your routine.
4. Regular Monitoring: Continue to monitor your BBT and menstrual cycle closely. This will help you and your doctor understand your ovulation patterns better.
5. Consultation with Specialists: If you continue to experience irregular cycles or have difficulty conceiving, consider consulting a fertility specialist. They can provide more targeted treatments, such as ovulation induction medications or assisted reproductive technologies if necessary.
6. Partner's Health: Don’t forget to consider your partner’s health as well. At 59, your husband should also have a health check-up to ensure his sperm quality is optimal for conception.
In conclusion, while conceiving at 39 with irregular cycles can be more challenging, it is certainly possible. By following your doctor's advice, maintaining a healthy lifestyle, and closely monitoring your cycles, you can improve your chances of becoming pregnant. Remember to stay positive and patient throughout this process, as it can take time. Good luck!
