Frequent urination, persistent urge to urinate, and pain between the anus and the testicles?

Hello doctor, I used to experience frequent urination around 8 or 9 PM, always feeling the urge to urinate and that my bladder wasn't completely empty.
Additionally, I would have pain in the perineal area between the anus and the testicles while sleeping at night.
I also felt some bloating and pain in the lower left abdomen.
This wasn't a daily occurrence; it happened occasionally, about a few days each month, and the frequency was low.
Recently, I had sexual intercourse with an online friend using a condom.
However, in the past few days, I've suddenly experienced frequent urination throughout the day, with a persistent urge to urinate and a slight burning sensation in the urethra.
I've been intentionally drinking more water, and my urine is clear and colorless.
During urination, I feel a strange sensation in the perineal area just in front of the anus, but it’s not painful.
I also have a slight bloating sensation in the lower abdomen.
Could this be prostatitis?

Hello: The diagnosis of prostatitis requires examination and testing; however, the symptoms may not be typical, as there can be abdominal discomfort (which occurs for a few days each month).
It may still be necessary to seek medical attention, and only after normal test results can one feel at ease.
Thank you for your inquiry.

Frequent urination and pain in the perineal area, particularly between the anus and testicles, can be indicative of several underlying conditions, including prostatitis, urinary tract infections (UTIs), or other urological issues. Given your symptoms, it is essential to consider a few key points.

Firstly, prostatitis, which is the inflammation of the prostate gland, can indeed cause symptoms such as frequent urination, a feeling of incomplete bladder emptying, and discomfort in the perineal region. This condition can be acute or chronic. Acute prostatitis typically presents with severe symptoms, including fever and chills, while chronic prostatitis may have milder symptoms that can fluctuate over time, as you described.

Your mention of experiencing pain in the perineal area, particularly when urinating, alongside the sensation of urgency and incomplete voiding, aligns with common symptoms of prostatitis. Additionally, the left lower abdominal discomfort you are experiencing could be related to pelvic floor tension or referred pain from the prostate.

Another possibility is a urinary tract infection (UTI), which can cause similar symptoms, including frequent urination, urgency, and discomfort during urination. The fact that your urine is clear and colorless is generally a good sign, as it indicates proper hydration and may suggest that there is no significant infection at this moment. However, UTIs can sometimes present with mild symptoms or may not always produce cloudy or discolored urine.

It is also worth noting that sexual activity, even with the use of condoms, can sometimes lead to irritation or inflammation in the urogenital area, which might contribute to your symptoms. If there was any trauma or irritation during intercourse, it could potentially lead to discomfort in the perineal region.

Given the intermittent nature of your symptoms and their recent increase in frequency, it is advisable to consult a healthcare professional for a thorough evaluation. A urologist can perform a physical examination, including a digital rectal exam, and may recommend urine tests or imaging studies to rule out infections, inflammation, or other urological conditions.

In the meantime, here are some general recommendations that may help alleviate your symptoms:
1. Hydration: Continue to drink plenty of water to help flush out your urinary system.

2. Avoid Irritants: Limit caffeine, alcohol, and spicy foods, as these can irritate the bladder and exacerbate urinary symptoms.

3. Warm Compress: Applying a warm compress to the pelvic area may help relieve discomfort.

4. Pelvic Floor Exercises: Engaging in pelvic floor exercises (Kegel exercises) may help strengthen the muscles in that area and alleviate some discomfort.

In conclusion, while your symptoms could be related to prostatitis or a UTI, a definitive diagnosis can only be made through a medical evaluation. It is crucial to seek medical attention to address your symptoms effectively and to rule out any serious underlying conditions.
