Discomfort Between Testicles and Anus: Possible UTI Causes - Urology

There is a feeling of swelling, warmth, and discomfort between the testicles and the anus?

About two weeks ago, after masturbating, I felt a swollen, warm, and uncomfortable sensation in the area between my testicles and anus, similar to an inflammatory feeling.
My personal history of urinary tract infections includes experiencing hematuria for the first time during college about four years ago.
I visited a urology clinic, where urine tests showed bacteria in my urine.
After taking the prescribed medication, follow-up tests showed normal results.
The second occurrence was during my military service (though I'm not entirely sure, as I had a fever at that time).
The most recent discomfort began around August 19, after I had masturbated approximately every other day from August 12 to August 18.
I typically masturbate about once a week (occasionally twice a week), but this time was the most uncomfortable.
The discomfort was located between my testicles and anus, accompanied by a burning and painful sensation while urinating.
After masturbating, I also experienced pain during urination (this has been happening for about a year, where I feel pain during urination about half an hour after ejaculation, but subsequent urination does not cause pain).
Recently, I have also noticed some memory issues.
In terms of recent medical history, after August 19, I visited a general clinic where the doctor tested my urine and found an increased presence of bacteria and white blood cells, diagnosing me with a urinary tract infection.
After taking medication, I felt improvement and a sense of recovery (the doctor prescribed antibiotics, and my urine appeared orange).
Once the burning sensation during urination subsided, I stopped the medication for about a week, but the burning sensation returned along with the uncomfortable feeling between my testicles and anus.
That morning, after consuming cranberry juice, I noticed improvement by noon, and the discomfort in that area resolved.
Later that evening, I returned to the clinic for another urine test, which showed significant improvement, but bacteria were still present (the clinic advised me to go to a larger hospital for further examination).
Two days later, I went to National Taiwan University Hospital for testing, where my urine test results were normal, and both urine flow and bladder ultrasound were also normal (the doctors at NTU seemed to prescribe medication to relax my urethra).
After taking the medication from NTU, I returned to my normal physiological state.
Since August 19, I have not masturbated, but I have experienced erections.
This morning, I felt a burning sensation during urination and a similar feeling during bowel movements, reminiscent of the situation on August 19.
I visited a urology clinic again, where they also suggested a urinary tract infection but did not perform a urine test.
The doctor mentioned that this episode has lasted longer than previous ones, raising the possibility of prostatitis.
Should I consider masturbating once to clear that area of the prostate? Could my activities, such as jumping rope, Kegel exercises, and squats, have impacted this episode of illness?

Dr. Chen Jiansheng reply Urology

It sounds like prostatitis.
Treatment for prostatitis requires patience, and this condition is not affected by regular exercise.

The discomfort you're experiencing between the testicles and anus, particularly after masturbation, can indeed be concerning and may be related to a urinary tract infection (UTI) or other underlying conditions. Given your history of UTIs and the symptoms you've described, it is essential to consider several factors that could contribute to your current situation.

Firstly, the area between the testicles and anus is known as the perineum, and it is rich in nerve endings and blood vessels. Discomfort in this area can arise from various causes, including infections, inflammation, or even muscle tension. Your previous experiences with UTIs suggest that you may be more susceptible to urinary tract issues, which can manifest as pain or discomfort during urination, ejaculation, or even in the perineal region.

The symptoms you described, such as a burning sensation during urination and discomfort after ejaculation, could indicate inflammation of the prostate gland (prostatitis) or the presence of an infection. Prostatitis can cause pain in the perineum, lower abdomen, and even the lower back. It is also known to cause urinary symptoms, including urgency, frequency, and burning during urination. Given that you have experienced similar symptoms in the past, it is plausible that prostatitis could be a contributing factor.

Regarding your question about whether masturbation could help "clean out" the prostate, it's important to note that while ejaculation can sometimes relieve pressure and discomfort associated with prostatitis, it is not a guaranteed solution. In some cases, it may even exacerbate symptoms, especially if there is inflammation present. Therefore, it is advisable to approach this with caution and to consult with a healthcare professional before making any decisions about sexual activity during this time.

Your recent urinary symptoms and the fact that you have been active with exercises like jumping rope and Kegel exercises could also play a role in your discomfort. Kegel exercises are generally beneficial for pelvic floor health, but if done incorrectly or excessively, they might lead to muscle tension or discomfort in the pelvic region. It is essential to ensure that you are performing these exercises correctly and not overexerting yourself.

In light of your ongoing symptoms and the fact that you have had a prolonged period of discomfort, it would be prudent to follow up with a urologist or a healthcare provider specializing in male pelvic health. They may recommend further testing, such as a prostate exam, imaging studies, or additional urine tests, to rule out any serious conditions and to determine the best course of treatment.

In summary, while your symptoms may be related to a UTI or prostatitis, it is crucial to seek professional medical advice to obtain an accurate diagnosis and appropriate treatment. Avoid self-diagnosing or self-treating, as this could lead to complications or prolonged discomfort. Your health and well-being should always be the priority, and a healthcare provider can guide you through the necessary steps to address your concerns effectively.
