Intermittent pain in the left rib cage and difficulty breathing?
Hello Doctor, I have had a mild scoliosis issue since elementary school.
In college, I started experiencing intermittent pain in my left rib, especially noticeable when I inhale, which leads to difficulty breathing.
I recently went to the hospital for an examination, and they indicated that my left rib is more prominent.
However, the X-ray results showed that the degree of scoliosis is not severe enough to cause pain, so nothing was done about it.
I would like to ask if there could be other issues or how I should proceed.
Thank you!
Xiao Yi, 20~29 year old female. Ask Date: 2017/04/18
Dr. Li Yongheng reply Orthopedics
If the chest X-ray shows no abnormalities in the lungs or bones, there should be no major concerns.
The cause may be due to poor posture, improper exertion, or pressure from side sleeping.
It is recommended to continue monitoring and have regular outpatient follow-ups.
Wishing you a speedy recovery!

Left rib pain accompanied by breathing difficulties can be concerning and may arise from various underlying conditions. Given your history of mild scoliosis since childhood and the recent onset of rib pain, it is essential to consider multiple factors that could contribute to your symptoms.
Firstly, the pain you experience, particularly when inhaling, could be related to musculoskeletal issues. Conditions such as costochondritis, which is inflammation of the cartilage connecting the ribs to the sternum, can cause localized pain that worsens with deep breaths or certain movements. Additionally, muscle strain or tension in the intercostal muscles (the muscles between the ribs) can lead to discomfort, especially if you have been engaging in activities that require repetitive upper body movements or if you have poor posture.
Your scoliosis may also be a contributing factor. While your X-ray results indicate that the curvature is not severe enough to cause significant pain, it can still lead to uneven distribution of stress on the rib cage and surrounding muscles. This uneven stress may result in discomfort, particularly if certain positions or movements exacerbate the pain. Furthermore, the rib cage's structure can be affected by scoliosis, potentially leading to restricted lung capacity or altered breathing mechanics, which could explain the breathing difficulties you are experiencing.
Another possibility to consider is the presence of a respiratory condition. Although less likely given your description, conditions such as pleurisy (inflammation of the lining of the lungs) or even a mild respiratory infection could lead to pain and difficulty breathing. However, these conditions are often accompanied by other symptoms such as fever, cough, or significant shortness of breath.
Given your symptoms, it is advisable to pursue further evaluation. A thorough physical examination by a healthcare professional, possibly a pulmonologist or an orthopedic specialist, may provide additional insights. They may recommend imaging studies beyond X-rays, such as a CT scan, to assess the rib cage and surrounding structures more comprehensively. Additionally, pulmonary function tests could help evaluate your lung capacity and function, especially if breathing difficulties persist.
In terms of management, if the pain is indeed musculoskeletal, treatments may include physical therapy to strengthen the muscles around the rib cage and improve posture. Pain management strategies, such as nonsteroidal anti-inflammatory drugs (NSAIDs), may also be beneficial. If your symptoms are related to muscle strain, rest and avoiding aggravating activities can help facilitate recovery.
In conclusion, while your scoliosis and rib pain may be interconnected, it is crucial to rule out other potential causes. A multidisciplinary approach involving physical therapy, pain management, and possibly further diagnostic imaging will help address your symptoms effectively. Regular follow-ups with your healthcare provider will ensure that any changes in your condition are monitored and managed appropriately.
