After nasal surgery, there may be some yellow-green nasal discharge?
In the past two to three years, every time I catch a cold, I experience nasal congestion, facial swelling, post-nasal drip, and thick, dark nasal mucus (located in the nasopharynx).
Each episode lasts about one to three months, occurring approximately twice a year.
Six months ago, I had an X-ray at National Taiwan University Hospital, which showed no significant issues with my sinuses.
However, I underwent surgery for inferior turbinate reduction, septoplasty for a deviated septum, and posterior nasal nerve section.
After the surgery, the feelings of nasal congestion and facial swelling have mostly disappeared, but I still have yellow-green nasal discharge (sometimes clear, but always somewhat sticky).
When I rinse my nose, I often expel thick, deep yellow secretions (or suction them out), and I frequently blow my nose.
This has caused discomfort in my eardrum and Eustachian tube.
Last week, I originally scheduled an appointment with my surgeon, but the doctor was on leave, and a resident physician saw me instead.
The resident mentioned that there is no further structural improvement possible and advised me to continue nasal irrigation.
Later, I visited another hospital and was told I might have sinusitis (I have received differing opinions from various doctors over the years).
I would like to know if it is appropriate to return for a follow-up with my original surgeon, considering I just saw the resident last week.
Additionally, I have a lump in front of my right ear, which seems to be a lymph node that I discovered years ago after experiencing swelling and pain during a cold.
It is less than 1 cm in diameter, has a smooth surface, and appears movable.
It has been present for about six to seven years.
Could this be related to my aforementioned conditions (as the nasal discharge is more severe from my right nostril)? Thank you, doctor.

Hello, if there is no improvement, it is recommended to return to the original surgeon for further examination.
As for the lymph nodes in the face, they may not necessarily be directly related to the nose.
Wishing you peace and health.
Sincerely, Dr.

Post-nasal surgery, particularly involving procedures like turbinate reduction, septoplasty, and nerve ablation, can lead to a variety of symptoms, including persistent yellow-green mucus. This symptom can be indicative of several underlying issues, including sinusitis, which is an inflammation of the sinus cavities that can occur after surgery or as a result of chronic nasal conditions.
The presence of yellow-green mucus is often associated with infection, particularly bacterial sinusitis. However, it can also occur in cases of viral infections or even allergic rhinitis. In your case, the fact that you have experienced these symptoms for several years, particularly after colds, suggests a chronic condition that may not be fully resolved by the surgical interventions you have undergone. The surgery may have alleviated some structural issues, such as nasal obstruction and facial swelling, but it does not necessarily eliminate the underlying causes of mucus production or sinus inflammation.
The yellow-green color of the mucus typically indicates the presence of white blood cells, which are part of the body’s immune response to infection. The thick, sticky nature of the mucus can also be a sign of inflammation in the nasal passages or sinuses. The fact that you are experiencing post-nasal drip and the sensation of mucus accumulation in the throat further supports the possibility of ongoing sinus issues.
Regarding your question about returning to your original surgeon, it is advisable to do so, especially if you feel that your symptoms have not improved or have worsened. Your original surgeon will have the most comprehensive understanding of your surgical history and can provide insights into whether your current symptoms are related to the surgery or if they indicate a new or ongoing issue, such as sinusitis. It is also important to bring any new symptoms or concerns to their attention, including the lump near your right ear, which could be a lymph node that has been reactive due to chronic inflammation or infection.
The lump you described, which has been present for several years and is movable and smooth, is likely a benign lymph node. However, it is essential to monitor any changes in size, tenderness, or associated symptoms, as these could warrant further evaluation. While it may not be directly related to your nasal symptoms, chronic inflammation in the nasal passages can sometimes lead to reactive lymphadenopathy in nearby areas.
In summary, your ongoing symptoms of yellow-green mucus, nasal congestion, and the presence of a lymph node should be evaluated by an ear, nose, and throat (ENT) specialist. They may recommend imaging studies, such as a CT scan of the sinuses, to assess for any signs of sinusitis or other complications. Additionally, they may suggest treatments ranging from nasal saline irrigation to antibiotics or other medications to manage inflammation and infection. It is crucial to address these symptoms promptly to prevent potential complications and improve your quality of life.
