Post-banding of internal hemorrhoids?

I would like to ask Dr.
Ke: First, on May 22, I underwent a second hemorrhoid ligation, and after 2 to 3 days, a soft, painless, and itch-free lump appeared at the anal opening.
I am certain that I only have internal hemorrhoids.
After taking a sitz bath, it noticeably enlarges, but it shrinks after resting.
What could this be? Second, after having a bowel movement, I always wash myself, but after a while, I sometimes feel moisture around the anus, and when I wipe, there is a yellowish discharge (similar to stool).
Is this due to dietary issues (I often drink cold beverages), a flare-up of hemorrhoids, or because I have irritable bowel syndrome? Because of this, I frequently check the toilet paper to see if there is any yellow discharge, and I worry that I might have obsessive-compulsive disorder.
Thank you.

1.
It could be external hemorrhoids, but a proper examination is necessary to confirm.

2.
As people age, the anal sphincter tone may decrease; however, at your age, it is likely too early for this to be a concern.
If your stools are looser, there is a possibility of mild leakage, but as long as there is no odor or contamination, it may not require attention.
Regarding whether your situation meets the diagnostic criteria for obsessive-compulsive disorder, it would be best to consult a psychiatrist for clarification.

After undergoing hemorrhoid ligation, it is not uncommon to experience a variety of symptoms as your body heals. Let's address your concerns one by one.

1. Soft Lump Near the Anus: The soft lump you noticed a few days after your second ligation could be a few things. It might be a thrombosed hemorrhoid, which occurs when a blood clot forms in the hemorrhoid, leading to swelling. This can happen even after ligation if there is still some engorgement of the surrounding tissues. Another possibility is a skin tag, which can develop as a result of the ligation procedure or from previous hemorrhoidal issues. These skin tags are generally harmless and can be soft and non-painful. If the lump persists or changes in size or characteristics, it would be wise to consult your healthcare provider for a thorough examination.

2. Post-Bowel Movement Discomfort: The sensation of moisture around the anus after bowel movements, along with the yellowish discharge, could be attributed to several factors. It might be related to residual stool that is not completely cleaned away, especially if you have a history of constipation or bowel irregularities, such as irritable bowel syndrome (IBS). The presence of moisture could also be due to anal irritation or inflammation, which can occur after hemorrhoid surgery. If you are experiencing this frequently, it may be beneficial to adjust your diet to include more fiber, which can help regulate your bowel movements and reduce straining. Additionally, drinking plenty of water can help soften stools.

Regarding your concern about the yellow discharge, it is essential to differentiate between normal post-surgical discharge and signs of infection. If the discharge has a foul odor, is accompanied by fever, or if you experience increased pain, these could be signs of an infection, and you should seek medical attention promptly.

3. Dietary Considerations: You mentioned that you often consume cold beverages. While cold drinks are not typically harmful, they can sometimes lead to digestive discomfort in sensitive individuals. If you suspect that your diet might be contributing to your symptoms, consider keeping a food diary to track what you eat and how it affects your bowel movements and overall comfort. This can help identify any potential triggers.

4. Psychological Concerns: Your anxiety about checking for yellow stains on toilet paper may indicate a heightened level of concern about your health, which can sometimes lead to obsessive behaviors. It’s essential to recognize that while being attentive to your body is important, excessive worry can lead to unnecessary stress. If you find that this behavior is impacting your daily life, it may be helpful to speak with a mental health professional who can provide strategies to manage anxiety.

In summary, while some post-operative symptoms are normal, it is crucial to monitor your condition closely. If you have persistent or worsening symptoms, or if you are concerned about any changes, do not hesitate to reach out to your healthcare provider for further evaluation and guidance. They can provide you with personalized advice based on your specific situation and medical history. Remember, your recovery is a process, and it’s important to give your body the time it needs to heal properly.
