Testicular and urinary issues?
Hello Doctor: I would like to ask you a question.
Ten years ago, I had an examination of my testicles because I felt a small lump the size of a grain of rice when I touched them.
The doctor performed a physical examination and an ultrasound, and he seemed to say there was no problem.
It has been a while, and I forgot what he told me.
Usually, there is no pain, but sometimes after hugging my girlfriend (with her on top while lying down), I feel a bit of pain afterward, although it doesn't happen every time.
Recently, I watched a program that introduced testicular cancer, and I have been feeling anxious about it.
In the past month, I have also experienced a sensation of incomplete urination, and after urinating, if I squat and then stand up, I feel like I have a slight leakage.
Additionally, when I urinate, it doesn't come out all at once; it comes out in two or three streams, and towards the end, it dribbles a bit, which feels like a drop.
I would like to know what problems I might be facing.
Thank you, Doctor.

Hello: Testicular cancer progresses rapidly and cannot be delayed for ten years.
It's advisable to have regular check-ups.
However, since you are experiencing urinary issues, you should drink plenty of water, engage in moderate exercise, and avoid alcoholic beverages and spicy foods.
If there is no improvement, please seek medical attention.
Wishing you good health.

Understanding testicular and urinary issues is crucial for maintaining reproductive and urinary health. Based on your description, it seems you are experiencing a combination of testicular discomfort and urinary symptoms that warrant further investigation.
Firstly, the presence of a small lump on the testicle that was previously evaluated with a physical examination and ultrasound is significant. While the initial assessment indicated no issues, it is essential to monitor any changes in size, shape, or sensation of the lump. Testicular lumps can be benign, such as epididymal cysts or varicoceles, but they can also be indicative of more serious conditions, including testicular cancer. The fact that you occasionally experience pain during certain physical activities, such as hugging your girlfriend, could suggest that the lump or surrounding tissue is sensitive or that there may be some underlying issue that requires further evaluation.
Regarding your urinary symptoms, the feeling of incomplete bladder emptying, the sensation of dribbling after urination, and the need to urinate in multiple streams can be associated with several conditions. These symptoms may indicate issues such as bladder dysfunction, urinary tract infections (UTIs), or even prostate-related problems, especially if you are male and of an age where prostate enlargement could be a factor.
The sensation of not fully emptying the bladder can be particularly concerning, as it may lead to urinary retention, which can increase the risk of infections and other complications. Additionally, the intermittent flow of urine and the feeling of dribbling could suggest that there is some obstruction in the urinary tract, which could be due to an enlarged prostate or other anatomical issues.
Given your history of testicular discomfort and the recent onset of urinary symptoms, it is advisable to seek a comprehensive evaluation from a urologist. This may include:
1. Physical Examination: A thorough examination of the testicles and prostate to assess for any abnormalities.
2. Ultrasound: A follow-up ultrasound may be warranted to evaluate the testicular lump further and to assess the urinary tract for any obstructions or abnormalities.
3. Urinalysis: A urine test can help identify any signs of infection, blood, or other abnormalities that may explain your urinary symptoms.
4. Urodynamic Studies: If bladder dysfunction is suspected, urodynamic testing can assess how well the bladder and urethra are functioning.
5. Cystoscopy: In some cases, a cystoscopy may be performed to visualize the inside of the bladder and urethra directly.
It is understandable to feel anxious about the possibility of serious conditions like testicular cancer, especially after seeing media coverage on the topic. However, early detection and treatment are key to managing any potential issues effectively.
In summary, your symptoms of testicular discomfort and urinary issues should not be ignored. A proactive approach involving a consultation with a urologist will help clarify the underlying causes and guide appropriate treatment. Remember, it is always better to address these concerns sooner rather than later to ensure your health and peace of mind.
