Red spots on the glans?
Hello Doctor: This past month, shortly after a sexual encounter, I experienced redness and pain in the foreskin, so I visited a urology clinic.
I was prescribed Doymycin pills and Mycomb topical medication.
A week later, I suddenly noticed that gentle pressure on the tip of the glans caused pain.
I decided to go to the hospital for a urine test, and the results came back normal.
That day, I also noticed some flaking on the glans.
I remember the doctor initially diagnosed me with balanoposthitis and prescribed Fusidic Acid topical ointment to apply twice daily.
However, after using it for a week, I recently discovered some red spots on the glans, and the area around the glans appears red as well, with one or two red spots.
Should I stop using this ointment or continue? I'm increasingly worried because now, sometimes at the beginning and end of urination, I feel pain, and gentle pressure on the tip of the glans also causes discomfort.

Hello: 1) For inflammation of the glans, applying ointment may lead to flaking in some individuals, which is generally not a major issue.
Please keep the area dry and clean with water.
Avoid applying the ointment too frequently.
Pay attention to your diet by avoiding alcohol, spicy, and irritating foods.
Ensure you get enough sleep.
If you have a foreskin, make sure to dry the area thoroughly after urination.
Inflammation of the glans can recur, so it's important to maintain vigilance to prevent reoccurrence.
2) Sometimes, there may be a painful sensation at the beginning and end of urination, and gentle pressure on the tip of the glans may also cause pain.
--> Drink plenty of water (at least 2500cc per day, avoid beverages).
If symptoms do not improve in 2-3 days, seek medical attention.
3) If you have engaged in unsafe sexual practices, please refer to http://www.isu.edu.tw/upload/04/7/news/postfile_60883.pdf.
Wishing you good health.

More Info
When it comes to understanding red spots on the glans (the head of the penis), it is essential to consider various potential causes and treatment options. Based on your description, it seems that you have been experiencing symptoms consistent with balanitis (inflammation of the glans) and possibly post-inflammatory changes, which can include redness, pain, and the appearance of red spots.
Possible Causes of Red Spots on the Glans
1. Balanitis: This is an inflammation of the glans, often caused by infections (bacterial, fungal, or yeast), irritants (such as soaps or lotions), or underlying skin conditions (like eczema or psoriasis). The symptoms you describe, including redness and pain, align with this condition.
2. Infections: Sexually transmitted infections (STIs) such as herpes, gonorrhea, or chlamydia can also cause redness and discomfort in the genital area. Although your urine tests came back normal, it might be worth considering a more comprehensive STI screening if you have had unprotected sexual encounters.
3. Allergic Reactions or Irritation: Sometimes, products like condoms, lubricants, or soaps can cause allergic reactions leading to redness and irritation.
4. Dermatological Conditions: Conditions such as lichen planus or psoriasis can manifest as red spots or lesions on the glans.
5. Post-Inflammatory Hyperpigmentation: After an episode of inflammation, the skin can develop red or dark spots as part of the healing process.
Treatment Options
1. Continue Medication: If your doctor prescribed Fusidic Acid ointment, it is generally effective against bacterial infections. However, if you notice worsening symptoms or new developments (like the appearance of red spots), it is crucial to follow up with your healthcare provider. They may need to reassess your condition and possibly change your treatment.
2. Avoid Irritants: Ensure that you are not using any products that could irritate the area. This includes soaps, lotions, or any topical products that are not specifically designed for sensitive areas.
3. Hygiene: Maintain good hygiene by gently cleaning the area with mild soap and water. Avoid scrubbing, as this can exacerbate irritation.
4. Avoid Sexual Activity: Until you have a clearer understanding of the cause of your symptoms, it may be wise to abstain from sexual activity to prevent further irritation or potential transmission of infections.
5. Consult a Specialist: If symptoms persist or worsen, consider seeing a urologist or dermatologist specializing in genital conditions. They may perform a physical examination and possibly a biopsy if there is a concern about a more serious condition.
When to Seek Immediate Care
- If you experience severe pain, swelling, or discharge from the penis.
- If you develop fever or systemic symptoms, which could indicate a more serious infection.
- If the red spots increase in number or change in appearance.
Conclusion
In summary, the red spots on the glans could be due to a variety of causes, primarily related to inflammation or infection. It is essential to monitor your symptoms closely and maintain communication with your healthcare provider. If you have concerns about the treatment or if your symptoms are not improving, do not hesitate to seek further medical advice. Your health and comfort are paramount, and a healthcare professional can provide the most accurate diagnosis and appropriate treatment plan tailored to your specific situation.
