Questions After Traditional Hemorrhoid Surgery
Hello Doctor: I underwent traditional hemorrhoidectomy on September 11, and now, almost a month later, I feel like I need to have a bowel movement every time I urinate.
There is also a constant leakage of fecal fluid from my anus, and I have to use cotton to absorb it, changing it several times a day.
This has made me hesitant to go far from home.
Are these symptoms normal after surgery? Is this fecal incontinence? I'm very worried; if it's not, how long will it take for these symptoms to improve? Thank you for helping me understand.

After undergoing hemorrhoid surgery, it is not uncommon for patients to experience a range of symptoms as part of the recovery process. Your concerns about feeling the urge to have a bowel movement every time you urinate, along with the leakage of mucus or stool from the anus, are valid and understandably distressing.
Firstly, it is important to recognize that surgical procedures, especially those involving the rectal area, can lead to temporary changes in bowel habits and sensations. The sensation of needing to have a bowel movement when urinating can occur due to the proximity of the urinary and digestive systems, as well as the surgical manipulation of tissues in that area. This phenomenon is sometimes referred to as "rectal urgency" and can be exacerbated by swelling or inflammation following surgery.
The leakage of mucus or stool, which you described as "糞水滲出," can also be a common postoperative symptom. After hemorrhoid surgery, the surgical site may still be healing, and it is possible for some discharge to occur as the body recovers. This discharge can be a combination of mucus and residual stool, especially if there is any irritation or sensitivity in the anal area.
Regarding your concern about fecal incontinence, it is essential to differentiate between temporary postoperative symptoms and more chronic issues. Fecal incontinence is characterized by an inability to control bowel movements, leading to involuntary leakage of stool. If you are experiencing only minor leakage or mucus discharge without a complete loss of control over bowel movements, it may not be classified as fecal incontinence. However, if you find that you are unable to control your bowel movements at all, it is crucial to consult your surgeon or a gastroenterologist for further evaluation.
As for the duration of these symptoms, recovery times can vary significantly from person to person. Generally, it may take several weeks for the surgical site to heal adequately, and during this time, it is not unusual to experience discomfort, changes in bowel habits, and some degree of discharge. Most patients see gradual improvement over time, but if your symptoms persist beyond a month or worsen, it is advisable to seek medical attention.
In the meantime, here are a few tips that may help alleviate some of your symptoms:
1. Dietary Adjustments: Ensure you are consuming a high-fiber diet to promote regular bowel movements and prevent constipation. Staying hydrated is also crucial.
2. Hygiene: Maintain good hygiene in the anal area to prevent irritation. Gentle cleansing with warm water and avoiding harsh soaps can help.
3. Avoid Straining: When using the bathroom, try to avoid straining, as this can exacerbate symptoms and delay healing.
4. Follow-Up Appointments: Keep in touch with your surgeon or healthcare provider. They can provide personalized advice and assess your recovery progress.
5. Pelvic Floor Exercises: If appropriate, pelvic floor exercises may help strengthen the muscles around the rectum and improve control.
In conclusion, while your symptoms may be concerning, they can be part of the normal healing process after hemorrhoid surgery. However, it is essential to maintain open communication with your healthcare provider to ensure that your recovery is on track and to address any ongoing concerns. If you experience significant changes or worsening symptoms, do not hesitate to seek medical advice.
