Itchy and painful penis without discharge or redness?
Hello Doctor, I started experiencing some itching at the base of my penis three weeks ago.
Now, I sometimes feel pain while urinating, which is quite bothersome.
There is no redness or purulent discharge.
I thought it would resolve itself in a few days, but it hasn't.
It has been two months since my last sexual activity, and I'm unsure of the cause.
I'm currently in Japan and won't be returning to Taiwan for a long time.
I'm not sure what to do.
Please advise, thank you.
Tony

More Info
Hello Tony,
I understand that you're experiencing discomfort at the base of your penis, characterized by itching and occasional pain during urination. This can be quite distressing, especially when there are no visible signs like redness, swelling, or discharge. Given that you haven't had sexual activity in the last two months, it’s important to consider several potential causes for your symptoms.
Possible Causes
1. Irritation or Allergic Reaction: Sometimes, irritation can occur from soaps, detergents, or personal care products. If you've recently changed any products, this could be a factor. Allergic reactions can also lead to localized itching and discomfort.
2. Infections: While you mentioned no discharge, it’s still possible to have a mild infection. Conditions such as a yeast infection or balanitis (inflammation of the glans) can cause itching and discomfort. Even without visible symptoms, these infections can still cause pain during urination.
3. Dermatological Conditions: Skin conditions like eczema or psoriasis can also affect the genital area, leading to itching and discomfort. These conditions may not always present with obvious symptoms like redness or swelling.
4. Sexually Transmitted Infections (STIs): Although you haven't had sexual activity recently, some STIs can remain asymptomatic for a period. It's important to consider this possibility, especially if you have had unprotected sex in the past.
5. Phimosis or Tight Frenulum: If you are uncircumcised, a tight foreskin can cause discomfort and pain during urination or sexual activity. This can lead to irritation and itching at the base of the penis.
Recommendations
1. Maintain Hygiene: Ensure that you are keeping the area clean and dry. Use mild, unscented soap and avoid any harsh products that could irritate the skin.
2. Avoid Irritants: If you suspect an allergic reaction, try to identify and avoid any potential irritants, including soaps, lotions, or laundry detergents.
3. Over-the-Counter Treatments: You may consider using over-the-counter antifungal creams if you suspect a yeast infection. However, if the symptoms persist or worsen, it’s crucial to seek medical advice.
4. Hydration and Diet: Staying hydrated can help with urinary discomfort. Additionally, a balanced diet can support your immune system.
5. Consult a Healthcare Provider: Since you are currently in Japan, it would be advisable to consult a local healthcare provider. They can perform a physical examination and may recommend tests to rule out infections or other underlying conditions. If language is a barrier, consider seeking out clinics that cater to expatriates or have English-speaking staff.
6. Avoid Sexual Activity: Until you have a clearer understanding of your symptoms, it may be wise to avoid sexual activity to prevent further irritation or potential transmission of any infections.
Conclusion
While your symptoms may not seem severe, they are certainly uncomfortable and warrant attention. It’s essential to address these symptoms promptly to avoid any complications. If you find that your symptoms do not improve within a week or two, or if they worsen, please seek medical attention as soon as possible.
Take care, and I hope you find relief soon.
Best regards,
