Grayish White Wrinkled Skin on the Glans: Causes and Next Steps - Dermatology

There is a patch of grayish-white wrinkled skin on the glans?

Hello doctor, I would like to ask about a patch of grayish-white wrinkled skin on the glans.
When I pinch it, it feels somewhat elevated, similar to pinching skin.
I noticed a small thick grayish-white patch about six months ago.
I went to a urologist for an examination, and the doctor suggested it might be an early stage of genital warts.
I then started using Aldara and have been applying it for about 11 weeks.
The original grayish-white patch has not developed into an ulcer; it has only transformed into a grayish-white area (although some ulcers have appeared around it).
Now, that area has become a patch of grayish-white wrinkled skin.
I also went to a sexually transmitted disease prevention clinic for another examination, and the physician there believes this is not genital warts.
What could this skin issue be? Should I continue seeing a urologist, or should I consult a dermatologist?

Dr. Wu Yuxin reply Dermatology

Medical regulations state that "a physician must personally examine a patient before providing treatment or prescribing medication..." Therefore, online diagnosis is also not permitted.
If you are experiencing significant distress, it is advisable to seek medical attention promptly.

More Info

The condition you are describing, characterized by grayish-white, wrinkled skin on the glans, can be concerning and warrants careful evaluation. Based on your description and the fact that you have already consulted with a urologist and a sexually transmitted disease (STD) clinic, it seems that you are taking the right steps to address this issue. However, the persistence of the symptoms and the appearance of ulcers around the area suggest that further investigation may be necessary.

Possible Causes
1. Lichen Sclerosus: This is a chronic skin condition that can affect the genital area, leading to white patches and wrinkled skin. It is more common in women but can occur in men as well. It may cause discomfort, itching, and can lead to scarring if untreated.

2. Psoriasis: This autoimmune condition can also manifest in the genital area, leading to red, scaly patches that may appear white or silvery. It can be itchy and may require specific treatments.

3. Balanitis: This is an inflammation of the glans that can be caused by infections, irritants, or skin conditions. It can lead to changes in skin texture and color.

4. Dermatitis: Contact dermatitis from soaps, lotions, or other irritants can cause changes in the skin's appearance, including wrinkling and discoloration.

5. Sexually Transmitted Infections (STIs): While you mentioned that the doctors do not believe this is related to HPV (the virus that causes genital warts), other STIs can also cause changes in the skin of the genital area.

Next Steps
Given that you have already been evaluated by a urologist and a clinic specializing in STIs, it may be beneficial to consult a dermatologist who specializes in skin conditions affecting the genital area. A dermatologist can provide a more comprehensive assessment and may perform a skin biopsy if necessary to determine the exact nature of the skin changes.

Treatment Options
The treatment will depend on the underlying cause of the symptoms. Here are some general approaches based on potential diagnoses:
- For Lichen Sclerosus: Topical corticosteroids are often the first line of treatment to reduce inflammation and symptoms. In some cases, other immunosuppressive agents may be considered.

- For Psoriasis: Treatments may include topical treatments, phototherapy, or systemic medications depending on the severity.

- For Balanitis: Treatment may involve antifungal or antibacterial medications if an infection is present, along with proper hygiene practices.

- For Dermatitis: Identifying and avoiding irritants is crucial, along with the use of topical steroids or emollients to soothe the skin.

General Advice
1. Avoid Irritants: Be cautious with soaps, lotions, and other products that may irritate the area. Opt for gentle, fragrance-free products.

2. Maintain Hygiene: Keep the area clean and dry to prevent infections.

3. Monitor Symptoms: Keep track of any changes in the appearance of the skin, as well as any new symptoms such as pain, itching, or discharge.

4. Follow-Up: Regular follow-ups with your healthcare providers are essential to monitor the condition and adjust treatment as necessary.

In conclusion, while it is good that you have sought medical advice, further evaluation by a dermatologist may provide clarity on the condition and appropriate treatment options. It is important to address any concerns promptly to prevent complications and ensure the best possible outcome for your skin health.
