Regarding the glans penis?
Hello doctor, about two years ago I had my first experience.
At that time, I had phimosis, and even when erect, I could only see a little bit of the glans.
Later, I went to see a doctor, and retracting the foreskin behind the glans was very painful.
I requested a circumcision, and within a few days, I was able to retract it to the coronal sulcus.
After that, sometimes I would let the foreskin cover the glans, only occasionally exposing it during bathing to let it breathe.
Until this year, I noticed that I ejaculate very quickly, so I kept the glans exposed to increase friction, but I still feel like I ejaculate too quickly.
I want to know how to determine if my glans has keratinization (which may make it less sensitive).
Eventually, the foreskin could no longer cover the glans, and it remained exposed.
I wash it thoroughly every day and compare myself to the male actors in adult films.
I have noticed that before I am erect, there is a ring of deep purple veins around the coronal sulcus.
When I am erect and very excited while watching adult films, the entire glans turns deep purple (gradually the whole head becomes deep purple) instead of flesh-colored.
Could this be the reason I ejaculate quickly (commonly known as premature ejaculation)? What solutions do I have? Thank you.

Hello: The foreskin may cause sensitivity of the glans when stimulated, but this is not always the case.
Some say that circumcision can be more comfortable, but it may also lead to earlier ejaculation.
There are many reasons for this.
It is still recommended that you visit a urology department for evaluation.
Wishing you good health.

More Info
Understanding glans sensitivity and premature ejaculation (PE) is a multifaceted issue that can be influenced by anatomical, physiological, and psychological factors. Based on your description, it seems you have experienced a range of concerns related to your glans sensitivity and the speed of ejaculation, which can be distressing.
Glans Sensitivity and Keratinization
The glans penis is typically sensitive due to the high concentration of nerve endings. However, if the glans is frequently covered by the foreskin, it may not experience the same level of stimulation as it would if it were exposed. Over time, this can lead to a process known as keratinization, where the skin on the glans becomes thicker and less sensitive. This is a natural protective response, but it can also reduce sensitivity, potentially leading to premature ejaculation.
To determine if keratinization has occurred, you can look for signs such as:
1. Texture Change: The skin on the glans may appear thicker or rougher than usual.
2. Color Change: The glans may appear less pink and more pale or white due to the buildup of skin cells.
3. Sensitivity: You may notice a decrease in sensitivity during sexual activity or stimulation.
If you suspect keratinization, it may be beneficial to consult a urologist or dermatologist who specializes in male sexual health. They can provide a thorough examination and recommend appropriate treatments.
Premature Ejaculation
Premature ejaculation is defined as ejaculation that occurs with minimal sexual stimulation before, on, or shortly after penetration, and it can be influenced by various factors:
1. Psychological Factors: Anxiety, stress, and performance pressure can significantly affect sexual performance and lead to premature ejaculation.
2. Physiological Factors: Conditions such as hyper-sensitivity of the glans, hormonal imbalances, or neurological issues can contribute to PE.
3. Lifestyle Factors: Substance use, including alcohol and recreational drugs, can also impact sexual function.
Solutions and Management
1. Desensitizing Creams: Over-the-counter creams or sprays that contain numbing agents (like lidocaine) can help reduce sensitivity and prolong ejaculation. These should be used cautiously and according to instructions to avoid affecting your partner's sensitivity.
2. Behavioral Techniques: Techniques such as the "stop-start" method or the "squeeze" technique can help you gain better control over ejaculation. These methods involve pausing stimulation before ejaculation or applying pressure to the base of the penis to delay climax.
3. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through Kegel exercises can improve control over ejaculation. These exercises involve repeatedly contracting and relaxing the muscles used to stop urination.
4. Counseling or Therapy: If psychological factors are contributing to your premature ejaculation, speaking with a therapist or counselor who specializes in sexual health can be beneficial. Cognitive-behavioral therapy (CBT) can help address anxiety and improve sexual confidence.
5. Medication: In some cases, medications such as selective serotonin reuptake inhibitors (SSRIs) or other prescription treatments may be recommended by a healthcare provider to help delay ejaculation.
6. Consultation with a Urologist: Given your history of phimosis and sensitivity issues, it may be wise to consult a urologist. They can assess your condition and recommend treatments tailored to your specific needs.
Conclusion
It's important to remember that you're not alone in experiencing these issues, and many men face similar challenges. Open communication with your partner and seeking professional help can lead to effective solutions. Addressing both the physical and psychological aspects of glans sensitivity and premature ejaculation can significantly improve your sexual health and overall well-being.
