Two surgeries for retinal detachment have resulted in the reappearance of a small tear?
Hello, Doctor.
My situation is that for more than two years, I have undergone two surgeries on my right eye due to retinal detachment.
The first surgery was a scleral buckle procedure, and a year later, I experienced a re-detachment and underwent a vitrectomy.
After a long recovery and follow-up period, I now have regular monthly check-ups at an ophthalmology clinic.
I am very afraid of experiencing another detachment.
It has been almost a year since my second surgery.
1.
I currently have six or so persistent floaters in my right eye.
Is this normal?
2.
In the past two or three days, I have noticed small flashes of light in the lower part of my right eye.
I went for a follow-up appointment last night, and the doctor informed me that there is a new small tear, but it is minor and can be treated with laser therapy.
However, I still need to be cautious.
This morning, I noticed that the flashes in the area where I originally saw them are still present.
Is this normal? Could it be because the area has not healed yet after the laser treatment? Will that flash gradually disappear? I am really, really scared.
The doctor told me to return for a follow-up in a month after the laser treatment.

Hello: 1.
Floaters typically do not disappear and are considered normal.
2.
After laser treatment, flashes may not immediately resolve because the laser targets the area surrounding the tear, so the tear itself may still exist, and flashes may still occur.
If the situation does not worsen, there is no need to be alarmed; regular follow-up visits are sufficient.

Hello! Thank you for sharing your concerns regarding your eye health, particularly after experiencing retinal detachment and undergoing multiple surgeries. It's understandable to feel anxious about your condition, especially given the complexity of your situation. Let's address your questions and provide some additional context regarding retinal detachment and post-operative care.
1. Persistent Floaters: It's not uncommon for patients who have undergone retinal surgery to experience floaters, which are often described as small spots or threads that drift through your field of vision. These floaters can be a result of changes in the vitreous gel of the eye, especially after procedures like vitrectomy. While they can be bothersome, they are generally considered a normal part of the healing process. However, if the floaters suddenly increase in number or are accompanied by flashes of light, it’s essential to report these changes to your eye doctor immediately, as they could indicate a new issue.
2. Flashes of Light and New Tears: The appearance of flashes of light, particularly after surgery, can be concerning. These flashes often occur when the vitreous gel pulls on the retina, which can happen after surgery. The fact that your doctor identified a new small tear and treated it with laser therapy is a positive step. It's normal for the area to remain sensitive for a while after treatment, and the flashes may persist as the eye heals. Typically, these symptoms should gradually improve, but it’s crucial to monitor them closely. If the flashes do not diminish or if you notice any changes in your vision, you should seek further evaluation.
3. Healing Process: After laser treatment, it can take some time for the eye to heal fully. The presence of flashes may indicate that the retina is still adjusting after the procedure. It’s essential to follow your doctor's advice regarding follow-up appointments and to report any new symptoms. Your doctor will be able to assess the healing process and determine if any further intervention is necessary.
4. Anxiety and Monitoring: It's completely natural to feel anxious after experiencing retinal detachment and undergoing surgeries. Regular follow-ups are crucial for monitoring your eye health. Make sure to communicate openly with your eye care team about your fears and concerns; they can provide reassurance and guidance tailored to your situation.
5. General Advice: In addition to attending your follow-up appointments, consider maintaining a healthy lifestyle that supports eye health. This includes a balanced diet rich in antioxidants, staying hydrated, and protecting your eyes from excessive sunlight. Avoiding activities that could strain your eyes or increase the risk of injury is also advisable.
In summary, while the symptoms you're experiencing can be concerning, they are not uncommon after retinal surgery. Regular monitoring and open communication with your healthcare provider are key to managing your condition. If you have any further questions or if your symptoms change, don’t hesitate to reach out to your eye doctor for guidance. Your health and peace of mind are paramount, and your medical team is there to support you through this journey.
