Redness, itching, and erosion of the glans penis?

Hello Doctor: After sexual intercourse, my glans became itchy, red, and showed signs of erosion.
After applying a German topical ointment for a few days, it gradually improved, and the glans returned to its normal state.
However, after a certain instance of masturbation, a few days later, there were no symptoms, but soon after, I noticed white, net-like flakes that could be rubbed off on the glans.
Later, the foreskin also became locally red, and it felt like it was about to become red, itchy, and eroded again.
Doctor, is this a fungal infection or a bacterial infection? Is the broad-spectrum topical ointment effective?

Niming, 30~39 year old female. Ask Date: 2018/01/23

Dr. Du Shixi reply Urology

Hello: Once the ointment has been applied, do not apply it again.
Keeping the area clean and dry (which is extremely important) is key.
Currently, your description indicates that it has not improved; there may be a fungal infection or a bacterial infection—both are possible, and it is also possible to have both infections.
Please be cautious regarding any sexual activity.
Wishing you good health.

More Info

Understanding the symptoms of red, itchy, and eroded glans (the head of the penis) is crucial for diagnosing and treating potential infections or irritations. Based on your description, it seems you are experiencing symptoms that could be indicative of either a fungal infection (such as candidiasis) or a bacterial infection, possibly exacerbated by irritation or friction from sexual activity or masturbation.

Possible Causes
1. Fungal Infections: The presence of white, flaky patches that can be easily rubbed off, along with redness and itching, suggests a possible yeast infection, commonly caused by *Candida albicans*. This type of infection is more prevalent in warm, moist areas and can occur after sexual activity, especially if there is a disruption in the normal flora of the area.

2. Bacterial Infections: Bacterial infections can also cause similar symptoms, particularly if there is an imbalance in the skin's natural flora or if there has been any trauma to the area. Conditions such as balanitis (inflammation of the glans) can arise from bacterial infections, leading to redness, swelling, and discomfort.

3. Irritation or Allergic Reactions: Sometimes, irritation from soaps, lotions, or even condoms can lead to symptoms like redness and itching. Allergic reactions to certain materials or products can also manifest in similar ways.

4. Sexually Transmitted Infections (STIs): Certain STIs can present with symptoms of itching, redness, and discharge. If there is any possibility of exposure to STIs, it is essential to consider this in the differential diagnosis.

Treatment Options
1. Topical Antifungals: If a fungal infection is suspected, over-the-counter antifungal creams (such as clotrimazole or miconazole) can be effective. These are typically applied directly to the affected area.

2. Bacterial Treatments: If a bacterial infection is suspected, a healthcare provider may prescribe topical or oral antibiotics, depending on the severity and type of infection.

3. Hydrocortisone Cream: For inflammation and irritation, a mild hydrocortisone cream may help reduce redness and itching. However, this should be used cautiously and not for extended periods without medical advice.

4. Avoiding Irritants: It is crucial to avoid any known irritants, including harsh soaps, scented products, or tight clothing that may exacerbate the condition.

5. Consultation with a Healthcare Provider: Given the recurring nature of your symptoms, it is advisable to consult a healthcare provider for a proper diagnosis. They may perform a physical examination and possibly take a culture to determine the exact cause of your symptoms.

Conclusion
While the topical cream you mentioned (德國利膚軟膏) may provide temporary relief, it is essential to identify the underlying cause of your symptoms for effective treatment. If the symptoms persist or worsen, or if you notice any unusual discharge, it is crucial to seek medical attention promptly. Early intervention can prevent complications and ensure appropriate management of your condition.
