There is a sensation of urgency at the urethral opening?

Hello doctor, recently I often feel the urge to urinate at the urethral opening about 10 minutes after urinating, but I don't have much sensation in my bladder.
When I go to the bathroom, I don't urinate a lot, but it's not the case that I feel this urge every time after urinating.
Sometimes, I don't feel anything until my bladder reaches a certain capacity before I need to urinate.
Could you please explain what these symptoms might be? This issue has been bothering me for a long time, and it often affects me when I travel.
Could it be a urinary tract infection?

Yu, 10~19 year old female. Ask Date: 2018/02/18

Hello: You need to check your urinary urgency at least to make a preliminary assessment.
Please avoid drinking beverages for now and try this for a few days.
Wishing you good health.

More Info

Understanding the urgency at the urethral opening can be quite complex, and it often involves a combination of physiological and psychological factors. Based on your description, it seems that you are experiencing a sensation of urgency at the urethral opening shortly after urination, even when your bladder does not feel full. This condition can be quite bothersome, especially when it interferes with your daily activities or travel plans.

Possible Causes
1. Overactive Bladder (OAB): One of the most common causes of urgency is Overactive Bladder, which is characterized by a sudden urge to urinate that may be difficult to control. Individuals with OAB may experience frequent urination, urgency, and sometimes urge incontinence. The sensation you feel at the urethral opening could be a result of involuntary bladder contractions.

2. Urinary Tract Infection (UTI): Another potential cause is a urinary tract infection. UTIs can lead to symptoms such as increased urgency, frequency, and discomfort during urination. Even if you do not have a significant amount of urine, the irritation caused by the infection can create a sensation of urgency.

3. Bladder Irritants: Certain foods and beverages, such as caffeine, alcohol, spicy foods, and artificial sweeteners, can irritate the bladder and lead to increased urgency. If you consume these regularly, it might be worth considering their impact on your symptoms.

4. Pelvic Floor Dysfunction: Issues with the pelvic floor muscles can also contribute to urinary urgency. If these muscles are too tight or not functioning properly, they can affect how the bladder and urethra work together.

5. Psychological Factors: Anxiety and stress can exacerbate urinary symptoms. If you find that your urgency worsens in stressful situations or when you are away from home, it might be related to psychological factors.

Concerns and Recommendations
Given that this issue has been persistent and is affecting your quality of life, it is important to consult a healthcare professional for a thorough evaluation. Here are some steps you can take:
1. Keep a Bladder Diary: Documenting your urination patterns, fluid intake, and any associated symptoms can provide valuable information for your healthcare provider. This diary can help identify triggers and patterns related to your urgency.

2. Urinalysis: A simple urine test can help rule out a urinary tract infection or other abnormalities. If a UTI is present, appropriate antibiotic treatment can alleviate your symptoms.

3. Pelvic Floor Assessment: If pelvic floor dysfunction is suspected, a referral to a physical therapist specializing in pelvic health may be beneficial. They can provide exercises and techniques to improve muscle function.

4. Lifestyle Modifications: Consider reducing or eliminating bladder irritants from your diet. Staying hydrated is important, but also pay attention to when and what you drink.

5. Medication: If your symptoms are diagnosed as OAB, medications such as anticholinergics or beta-3 adrenergic agonists may be prescribed to help manage urgency and frequency.

6. Behavioral Techniques: Techniques such as bladder training or scheduled voiding can help retrain your bladder and reduce urgency.

Conclusion
In summary, the sensation of urgency at the urethral opening shortly after urination can stem from various causes, including overactive bladder, urinary tract infections, bladder irritants, pelvic floor dysfunction, and psychological factors. It is essential to seek medical advice to determine the underlying cause and appropriate treatment. By keeping a bladder diary and undergoing necessary evaluations, you can work with your healthcare provider to develop an effective management plan that addresses your symptoms and improves your quality of life.
