When can rehabilitation begin?
Due to a motorcycle accident, I suffered a fracture of the coronoid process.
I underwent surgery on February 21 for internal fixation with a titanium alloy plate.
Yesterday, on March 2, the stitches were removed and the cast was taken off.
Can I start going to physical therapy now? However, I still have some bruising and swelling in my elbow.
Thank you!

You can currently start performing straightening and bending movements, but it is not advisable to lift heavy objects yet.
It will take about six weeks to heal, after which you can engage in more vigorous activities.

After undergoing surgery for an elbow injury, such as an olecranon fracture, the timing for starting rehabilitation can vary based on several factors, including the nature of the injury, the surgical procedure performed, and the individual's overall health. In your case, since you had surgery on February 21 and the stitches were removed on March 2, it is understandable that you are eager to begin rehabilitation.
Generally, it is advisable to start rehabilitation as soon as possible after surgery, provided that the surgical site is stable and there are no complications. Early rehabilitation can help prevent stiffness, maintain range of motion, and promote healing. However, it is crucial to consider the following points:
1. Consult Your Surgeon: The most important step is to consult with your orthopedic surgeon or the physician who performed the surgery. They will assess your healing progress and determine if your elbow is stable enough to begin rehabilitation exercises. They will also provide specific guidelines based on your individual case.
2. Signs of Healing: If you still have bruising and swelling, it may be a sign that your elbow is still healing. While some swelling is normal after surgery, excessive swelling can indicate complications. Your surgeon will evaluate whether the swelling has decreased sufficiently to allow for rehabilitation.
3. Rehabilitation Goals: The primary goals of rehabilitation after elbow surgery include restoring range of motion, improving strength, and regaining functional use of the arm. A physical therapist can design a rehabilitation program tailored to your needs, which may include gentle range-of-motion exercises, strengthening exercises, and functional activities.
4. Gradual Progression: Rehabilitation should be approached gradually. Initially, your therapist may focus on passive and active-assisted range-of-motion exercises to gently mobilize the elbow joint without putting excessive strain on the healing tissues. As your strength and range of motion improve, more active and resistive exercises can be introduced.
5. Avoiding Complications: Early movement is essential, but it is equally important to avoid overexertion. Engaging in activities that are too strenuous too soon can lead to complications such as re-injury or delayed healing. Your therapist will guide you on how to progress safely.
6. Follow-Up Appointments: Regular follow-up appointments with your surgeon are crucial to monitor your healing progress. They may perform imaging studies, such as X-rays, to ensure that the bone is healing correctly and that there are no signs of complications.
In summary, while it is generally beneficial to start rehabilitation soon after surgery, the decision should be made in consultation with your healthcare provider. They will assess your specific situation, including the degree of swelling and bruising, and provide you with a personalized rehabilitation plan. Starting rehabilitation at the right time can significantly enhance your recovery and help you return to your normal activities as quickly and safely as possible.
