Abscess after fistula surgery
Hello Doctor, I previously asked you about the small abscess that developed five weeks after my fistula surgery.
During my follow-up visit, the doctor mentioned that there was some residual mucus that wasn't cleaned out during the surgery, which caused this issue.
He also said that the tissue growing from the wound was healthy, and that the localized abscess would heal on its own, requiring only the application of a topical ointment and no sitz baths.
I would like to ask you: 1.
Will the small abscess really heal on its own? It has been a few days, and the abscess is still there, neither getting bigger nor smaller.
2.
I am still doing sitz baths and adding some water-soluble ointment; is this care appropriate? Thank you, Doctor!

Dr. Ke Fangxu reply Surgery
After fistula surgery, there may occasionally be mild skin infections around the wound.
In such cases, sitz baths combined with antibiotic treatment can gradually improve the condition.
Adding povidone-iodine to the water is acceptable, but the concentration should not be too high to avoid causing skin irritation.

After undergoing fistula surgery, it's not uncommon to experience complications such as the formation of a small abscess. Your concerns about the small abscess that has developed five weeks post-surgery are valid, and it's important to understand how to manage this situation effectively.
1. Will the small abscess heal on its own?
In many cases, small abscesses can resolve on their own, especially if they are not causing significant pain or discomfort, and if they are not increasing in size. The body's immune system often works to clear minor infections, and if the abscess is small and stable, it may indeed heal without intervention. However, it is crucial to monitor the abscess closely. If you notice any changes such as increased redness, swelling, pain, or if the abscess begins to grow, it is essential to contact your healthcare provider immediately. These could be signs of a more serious infection that may require medical treatment, such as drainage or antibiotics.
2. Is it okay to continue sitz baths with added saline?
Sitz baths can be beneficial for promoting hygiene and comfort in the perineal area after surgery. Adding saline to the sitz bath can help cleanse the area and may provide some soothing effects. However, it's essential to follow your doctor's advice regarding the frequency and duration of sitz baths. If your doctor has advised against them, or if you notice any irritation or worsening of the abscess after using sitz baths, it would be wise to discontinue them and consult your physician for further guidance.
Additional Care and Considerations
- Wound Care: Continue to keep the area clean and dry. If your doctor has recommended applying a topical ointment, ensure that you do so as directed. Avoid any harsh soaps or irritants that could exacerbate the situation.
- Signs of Infection: Be vigilant for signs of infection, which can include increased pain, swelling, warmth around the area, or fever. If you experience any of these symptoms, seek medical attention promptly.
- Follow-Up Appointments: Regular follow-ups with your healthcare provider are crucial in monitoring the healing process. If the abscess does not resolve or if you have ongoing concerns, your doctor may need to reassess the area and consider further interventions.
- Pain Management: If you are experiencing discomfort, over-the-counter pain relievers may help. However, consult your doctor before taking any new medications, especially if you are on other prescriptions.
- Diet and Hydration: Maintaining a healthy diet and staying well-hydrated can support your immune system and overall recovery. Foods rich in vitamins C and E, zinc, and protein can be particularly beneficial for wound healing.
In conclusion, while small abscesses can often resolve on their own, close monitoring and proper care are essential. If you have any doubts or if the situation changes, do not hesitate to reach out to your healthcare provider for advice. Your health and recovery are paramount, and proactive communication with your medical team will help ensure the best outcomes.
