There is some discomfort in the glans and foreskin?
Hello doctor, I recently underwent a double J stent procedure.
Three days after the stent was removed, I started to notice some itching around the glans and foreskin, and I observed some spots at the urethral opening.
Subsequently, during masturbation, I noticed some foul-smelling yellow-green discharge.
I have not undergone circumcision, but I do have some issues with phimosis, as my foreskin covers the glans when not erect, although I can expose about half of the glans when erect.
Could this be due to residual urine trapped under the foreskin causing smegma, leading to balanitis? Additionally, when cleaning the glans, should I retract the foreskin all the way to the coronal sulcus? I usually rinse the glans with cold water without using my hands to scrub, and I do not use any cleaning products.
Is my cleaning method incorrect?

Dr. Chen Jiansheng reply Urology
If you are experiencing symptoms of a urinary tract infection, please seek medical evaluation.
During the cleaning of the foreskin, gently retract it without force, apply soap lightly, and rinse with clean water.
If you are concerned about infection or find cleaning difficult, you may consider circumcision.
Thank you.

More Info
It sounds like you are experiencing some discomfort in the glans (the head of the penis) and foreskin after undergoing a double J stent procedure. This type of surgery can sometimes lead to irritation or inflammation in the genital area, especially if there are changes in hygiene practices or if there is residual urine that can lead to irritation.
Understanding Your Symptoms
1. Itching and Discomfort: The itching sensation you are experiencing could be due to several factors, including irritation from urine, residual urine trapped under the foreskin, or even a mild infection. The presence of yellow-green discharge may indicate a possible infection, such as balanitis (inflammation of the glans) or posthitis (inflammation of the foreskin).
2. Discharge: The yellow-green discharge you mentioned is particularly concerning. This could be a sign of an infection, especially if it has an odor. It's important to monitor this symptom closely, as it may require medical evaluation and treatment.
3. Hygiene Practices: You mentioned that you have not undergone circumcision and that your foreskin covers the glans when not erect. This can sometimes lead to a buildup of smegma (a combination of dead skin cells, oils, and moisture) if proper hygiene is not maintained.
Recommendations for Hygiene
- Cleaning Technique: It is generally recommended to gently retract the foreskin to expose the glans during cleaning. This allows for proper hygiene and helps prevent the buildup of smegma. You should aim to clean the area with warm water, and it is advisable to avoid using harsh soaps or cleaning products that can irritate the sensitive skin of the glans and foreskin.
- Frequency of Cleaning: Regular cleaning is important, especially if you are prone to retaining urine under the foreskin. It is advisable to clean the area at least once a day or after any activity that may lead to sweat or moisture accumulation.
- Temperature of Water: While using cold water is not harmful, many find that warm water is more effective for cleaning and can be more comfortable.
When to Seek Medical Attention
- If the itching and discomfort persist or worsen, or if you notice an increase in the discharge or any other concerning symptoms (such as swelling, redness, or pain), it is crucial to consult a healthcare provider. They may perform a physical examination and possibly recommend tests to rule out infections or other underlying conditions.
- If you suspect that the discharge is due to an infection, it is important to seek medical advice promptly. Infections can often be treated effectively with antibiotics, but they may lead to complications if left untreated.
Conclusion
In summary, the symptoms you are experiencing could be related to irritation or an infection following your surgery. Maintaining proper hygiene by gently retracting the foreskin and cleaning the area with warm water is essential. If symptoms persist or worsen, please consult a healthcare provider for a thorough evaluation and appropriate treatment. Your health and comfort are paramount, and addressing these symptoms early can help prevent further complications.
