Disability Assessment
I was diagnosed with polio-related muscle atrophy at a very young age, around 2 years old.
My legs are of different lengths, which affects my mobility to this day.
I want to apply for a disability certificate, but I'm unsure which department can assist me with the evaluation.
I haven't applied for the assessment because I don't know how to proceed.
Please advise.
Thank you!

Hello Mr.
Tian Sheng: If you are suffering from muscle atrophy due to poliomyelitis resulting in limb length discrepancy, it is recommended to consult your original attending physician or specialists in rehabilitation medicine or pediatric neurology for the assessment application.

To obtain a disability certificate for conditions related to polio, particularly if you have experienced muscle atrophy and mobility issues since childhood, you will need to follow a specific process that typically involves medical evaluation and documentation. Here’s a step-by-step guide to help you navigate this process:
1. Consult a Medical Professional: The first step is to consult with a healthcare provider who specializes in physical medicine and rehabilitation (PM&R), also known as physiatrists. They are trained to assess and manage disabilities related to musculoskeletal and neurological conditions, including those resulting from polio. If you have a primary care physician, they can also refer you to the appropriate specialist.
2. Medical Evaluation: During your appointment, the physician will conduct a thorough evaluation of your condition. This may include a physical examination to assess your muscle strength, range of motion, and functional abilities. They may also review your medical history, including your history of polio and any treatments you have undergone.
3. Documentation of Disability: The physician will document their findings in a medical report. This report should detail the extent of your disability, including any limitations in mobility, daily activities, and the impact of your condition on your quality of life. It is crucial that this documentation is comprehensive, as it will be used to support your application for a disability certificate.
4. Application for Disability Certificate: Once you have the necessary medical documentation, you can apply for a disability certificate. This process may vary depending on your location, but generally, you will need to submit your medical report along with an application form to the relevant government agency or department responsible for disability services. In the United States, this may be the Social Security Administration (SSA) or your state’s disability services office.
5. Follow-Up Assessments: In some cases, the agency may require additional assessments or evaluations by their own medical professionals. Be prepared to attend these appointments if requested. They may also ask for further documentation or clarification regarding your condition.
6. Await Decision: After submitting your application and any required documentation, there will be a waiting period during which the agency reviews your case. This can take several weeks to months, depending on the volume of applications they are processing.
7. Appeal if Necessary: If your application is denied, you have the right to appeal the decision. The appeals process typically involves submitting additional evidence or clarification regarding your condition. It may also include a hearing where you can present your case.
8. Consider Support Services: Throughout this process, consider reaching out to local disability advocacy organizations. They can provide guidance, support, and resources to help you navigate the application process and understand your rights.
In summary, obtaining a disability certificate for polio-related conditions involves consulting with a medical professional, obtaining thorough documentation of your disability, and submitting an application to the appropriate agency. It is essential to ensure that all medical evaluations and documentation are comprehensive and accurately reflect the impact of your condition on your daily life. If you have any further questions or need assistance, do not hesitate to reach out to local health services or disability advocacy groups for support.
