Ejaculation Responses and Erectile Function: Is It Normal? - Urology

Erect state

Every time I masturbate and feel very excited, just before ejaculation, I experience a reaction (penile contractions) and some clear fluid is released, but there is no ejaculation.
After this, my penis doesn't become completely flaccid; it just becomes a little softer until just before ejaculation (about 90%).
However, my level of excitement almost disappears, and stimulating the penis requires more stimulation to maintain a full erection.
Additionally, I experience a sense of fatigue.
After continuous stimulation, I become excited again, and the fatigue disappears.
Finally, just before ejaculation, my penis becomes very hard again until ejaculation occurs.
Is that process in between considered orgasm? Am I experiencing a medical issue? (I remember having that intermediate stage before, but I don't recall if I had those feelings of fatigue or the softening.)

Dr. Du Mingyi reply Urology

When nearing ejaculation, forcibly holding back or during sexual arousal, certain glands in the prostate and urethra secrete fluids for lubrication.
This is similar to how females also secrete fluids during foreplay, and these processes are all normal.

Understanding ejaculation responses and erectile function can be complex, and it's important to recognize that variations in sexual response are common and can be influenced by a variety of factors, including physical health, psychological state, and even situational context.

From your description, it seems that you experience a series of physiological responses during masturbation that include pre-ejaculatory fluid release, penile contractions, and changes in erectile firmness. The transparent fluid you mention is likely pre-ejaculatory fluid (also known as pre-cum), which is produced by the Cowper's glands and can occur during sexual arousal, even if ejaculation does not follow. This fluid serves to lubricate the urethra and neutralize acidity, preparing for potential ejaculation.

The sensations you describe—such as the penis becoming slightly softer after the initial excitement, requiring more stimulation to maintain a full erection, and experiencing a sense of fatigue—are not uncommon. This phase can be related to the refractory period, which is the time following ejaculation during which a man may find it difficult to achieve another erection or may experience a decrease in sexual arousal. However, it sounds like you are experiencing a unique pattern where the excitement diminishes before ejaculation, which can be influenced by various factors, including psychological stress, fatigue, or even hormonal fluctuations.

Regarding whether this process constitutes a climax, it is essential to differentiate between orgasm and ejaculation. Orgasm is often described as the peak of sexual pleasure, accompanied by intense sensations and often leading to ejaculation. However, it is possible to experience orgasm without ejaculation, particularly in cases of delayed ejaculation or certain medical conditions. The fatigue and need for increased stimulation you describe could indicate that your body is responding differently than it has in the past, which may be a normal variation or could suggest an underlying issue.

If you are concerned about these changes, it may be beneficial to consult with a healthcare professional, particularly a urologist or a sexual health specialist. They can provide a thorough evaluation, including a discussion of your medical history, any medications you may be taking, and lifestyle factors that could be affecting your sexual function. Additionally, they can help determine if there are any underlying conditions that need to be addressed, such as hormonal imbalances, psychological factors, or other health issues.

In summary, while the experiences you describe can be part of normal sexual function, changes in your sexual response can warrant further exploration. Open communication with a healthcare provider can help clarify any concerns and ensure that you maintain a healthy sexual life. Remember, sexual health is an important aspect of overall well-being, and seeking help is a positive step towards understanding and addressing any issues you may face.
