Prescription reimbursement issues with health insurance?
Hello Dr.
Hou, I have been experiencing long-term muscle and nerve pain in my shoulders and neck.
I visited an orthopedic doctor and took anti-inflammatory medications and muscle relaxants, but they haven't been very effective.
Sometimes I feel a bit better, but the pain returns after a while.
As I am a vegetarian, I read online that vegetarians are more prone to deficiencies in Vitamin B1 and B12.
Which specialist should I see to get prescriptions for these two vitamins? Are these medications covered by health insurance? Thank you for your assistance!
Mr. He, 50~59 year old female. Ask Date: 2018/09/09
Dr. Hou Fangmin reply Pharmacology
Hello Mr.
He: Vitamin B12 deficiency can occasionally occur in individuals using the diabetes medication metformin.
A blood test can determine if there is a deficiency.
Vegetarians are most likely to experience deficiencies in vitamins B1 and B12 due to dietary issues.
If supplementation is needed, B-complex vitamins can be purchased over the counter.
Additionally, in hospitals, B12 is used not only for complications arising from diabetes but also for pain related to neurological and neuromuscular conditions; however, most other uses are not covered by health insurance.
Wishing you peace and joy.

In Taiwan, prescription coverage for nutritional deficiencies, particularly for vitamins such as B1 (thiamine) and B12 (cobalamin), is an important topic, especially for individuals following a vegetarian or vegan diet. As you mentioned, vegetarians are at a higher risk for deficiencies in these vitamins due to their dietary restrictions, which can lead to various health issues, including neurological problems, anemia, and muscle weakness.
To address your specific concerns, it is advisable to consult a healthcare professional who specializes in nutrition or internal medicine. A physician in these fields can evaluate your symptoms and dietary habits comprehensively. They may recommend blood tests to assess your levels of B1 and B12, as well as other nutrients that might be deficient due to your vegetarian diet.
Once a deficiency is confirmed, the physician can prescribe the necessary supplements. In Taiwan, under the National Health Insurance (NHI) system, certain nutritional supplements may be covered if they are deemed medically necessary. However, coverage can vary based on specific circumstances, such as the severity of the deficiency and the physician's assessment. It is essential to discuss this with your doctor, who can provide guidance on whether the supplements you need will be covered by insurance.
In addition to seeking medical advice, it is also beneficial to consider dietary adjustments. Incorporating fortified foods, such as cereals and plant-based milk that are enriched with B12, can help mitigate the risk of deficiency. Foods like nutritional yeast and certain types of algae are also good sources of B12 for vegetarians. For B1, whole grains, legumes, nuts, and seeds are excellent sources.
Moreover, it is crucial to understand that while supplements can help address deficiencies, they should not replace a balanced diet. A well-rounded vegetarian diet that includes a variety of fruits, vegetables, whole grains, and legumes can provide many essential nutrients. If you are unsure about your dietary intake, consulting with a registered dietitian can provide personalized guidance tailored to your nutritional needs.
In summary, to address your concerns about B1 and B12 deficiencies, you should:
1. Consult a healthcare professional, preferably a specialist in nutrition or internal medicine, to evaluate your symptoms and dietary habits.
2. Discuss the possibility of blood tests to check for deficiencies.
3. Inquire about prescription coverage for necessary supplements under Taiwan's National Health Insurance.
4. Consider dietary adjustments to include fortified foods and other sources of these vitamins.
5. If needed, seek advice from a registered dietitian for personalized nutrition guidance.
Taking these steps can help ensure that you maintain optimal health and address any nutritional deficiencies effectively.
