Postoperative limitation of forearm supination following distal ulnar fracture?

Hello Dr.
Yeh,
On July 28th of this year, I was involved in a car accident that resulted in a distal ulnar fracture in my right arm.
After more than a month, I am still experiencing limited external rotation of my arm, with no significant improvement, while internal rotation is smooth and functions like a normal arm.
Could the limitation in external rotation be due to the ulnar fracture still not healing, or could it be caused by adhesions? I hope you can help clarify this for me.
Thank you.

Simon, 30~39 year old female. Ask Date: 2018/09/10

Dr. Ye Qingnian reply Orthopedics

Please go to the rehabilitation department for therapy; doing more rehabilitation will lead to improvement.

Hello! Thank you for your question regarding the limited supination after your distal ulnar fracture surgery. It's understandable to be concerned about your recovery and the functionality of your arm after such an injury.

After a distal ulnar fracture, it is common for patients to experience limitations in certain movements, particularly supination (the motion of rotating the forearm so that the palm faces up) and pronation (the opposite motion). The fact that you are experiencing limited supination while your pronation feels normal can be attributed to several factors.

1. Healing Process: The healing of the bone itself can significantly impact your range of motion. If the distal ulnar fracture has not fully healed, it can lead to stiffness and limitations in movement. The healing process can take several weeks to months, depending on the severity of the fracture, your age, and overall health. If the bone is still in the process of healing, it may restrict your ability to perform certain movements comfortably.

2. Soft Tissue Adhesions: Another possibility for your limited supination could be the formation of adhesions or scar tissue around the surgical site. After surgery, the body naturally forms scar tissue as part of the healing process. If this scar tissue is extensive or improperly formed, it can restrict movement and lead to stiffness in the joint and surrounding tissues. This is particularly common in the forearm, where multiple muscles and tendons are involved in the supination and pronation movements.

3. Muscle Weakness: Following an injury and subsequent immobilization, muscle atrophy can occur. If the muscles responsible for supination (such as the biceps brachii and the supinator muscle) have weakened due to disuse, this can also contribute to limited movement. Strengthening these muscles through physical therapy can help improve your range of motion.

4. Joint Mobility: The wrist and elbow joints play a crucial role in forearm rotation. If there are any restrictions or stiffness in these joints due to the injury or surgery, it can affect your ability to supinate. A thorough assessment of these joints may be necessary to determine if they are contributing to your limitations.

To address your concerns, I recommend the following steps:
- Follow-Up with Your Surgeon: It's essential to have a follow-up appointment with your orthopedic surgeon. They can assess the healing of your fracture through physical examination and imaging studies (like X-rays) to determine if the bone is healing properly.

- Physical Therapy: Engaging in a structured physical therapy program can be beneficial. A physical therapist can provide targeted exercises to improve your range of motion, strengthen the muscles involved in supination, and help reduce any adhesions that may have formed.

- Monitor Symptoms: Keep track of any changes in your symptoms. If you notice increased pain, swelling, or any other concerning signs, make sure to communicate this with your healthcare provider.

In summary, limited supination after a distal ulnar fracture can be due to a combination of factors, including incomplete healing, soft tissue adhesions, muscle weakness, and joint mobility issues. A comprehensive approach involving follow-up care and physical therapy will be essential in addressing these limitations and facilitating your recovery. Don't hesitate to reach out to your healthcare team for further evaluation and guidance. Best wishes for your recovery!
