Penile tear due to sexual intercourse?

Hello Doctor,
As the title suggests, I have a tear on the upper part of my foreskin due to sexual intercourse (possibly due to insufficient lubrication).
I have phimosis, but I can manually retract the foreskin to fully expose the glans both when flaccid and erect without experiencing pain.
A few days ago, after intercourse, the tip of my foreskin was red, swollen, and torn.
Currently, the redness and swelling have improved significantly and are similar to when I was uninjured, but the tear is still present.
I am cleaning the area twice daily.
Since I am working in China, I am only using Betadine for disinfection and applying Mentholatum, but this may be causing slight red spots and itching on the glans, making me want to retract the foreskin for thorough cleaning.
However, this causes the wound to reopen slightly.
Will this make it very difficult for the wound to heal? On the other hand, I am concerned about bacterial growth and infection if I do not clean the glans, and it is inconvenient to seek medical help here in China.

I would like to ask the doctor:
1.
Is there a better or more appropriate way to manage the wound?
2.
Can you recommend any ointments to accelerate wound healing? (The ointments available in China are labeled with generic names, making it hard for me to identify them, e.g., Fluocinolone Acetonide Cream)
3.
I currently feel some tissue hyperplasia at the tip of the foreskin, leading to a hardened sensation.
Once the wound heals, is it likely to tear again easily?
I really need your remote assistance and consultation.
Thank you very much.

Dr. Du Mingyi reply Urology

You only need to disinfect with povidone-iodine daily; there is no need to use ointments.
In fact, using ointments can make the wound less likely to dry out.
Please keep the wound clean and dry.
The tissue that has been injured will differ from normal tissue and will be harder.
As long as you avoid further injury during the healing period, it will heal properly; otherwise, it will continue to re-heal repeatedly.

Managing penile tear injuries resulting from sexual activity can be a delicate issue, particularly when it involves the sensitive skin of the penis and potential complications such as infection or delayed healing. Here’s a comprehensive overview of how to manage such injuries, along with some recommendations based on your situation.

1. Understanding the Injury
A penile tear, especially one located on the foreskin or glans, can occur due to insufficient lubrication, vigorous sexual activity, or anatomical issues such as phimosis (a condition where the foreskin cannot be easily retracted). The fact that you are experiencing redness and swelling indicates that there was some degree of trauma, but the absence of pain is a positive sign, suggesting that the injury may not be severe.

2. Cleaning and Care
You mentioned that you are cleaning the area twice daily, which is essential. Here are some steps to ensure proper care:
- Gentle Cleaning: Use mild soap and warm water to clean the area. Avoid harsh soaps or scrubbing, as this can irritate the skin further.

- Avoid Irritants: Be cautious with any topical products. While you are using iodine for disinfection, it can be drying. Consider switching to a saline solution or a mild antiseptic that is less irritating.

- Moisturization: After cleaning, applying a thin layer of a water-based lubricant or a non-irritating ointment (like petroleum jelly) can help keep the area moist, which is conducive to healing.

3. Recommended Ointments
For promoting healing, look for ointments that contain:
- Aloe Vera: Known for its soothing properties.

- Vitamin E: Can help with skin healing.

- Hydrocortisone Cream: A mild steroid that can reduce inflammation, but should be used sparingly and not for prolonged periods.

In terms of specific medications, if you have access to them, consider asking a local pharmacist for a topical antibiotic ointment (like Neosporin) that can help prevent infection while promoting healing.

4. Addressing the Growth and Sensitivity
The sensation of tissue growth or hardening at the injury site could be due to scar tissue formation. This is common after any injury, but it can lead to complications if the area is not allowed to heal properly. Here are some considerations:
- Avoid Excessive Manipulation: While it’s important to keep the area clean, avoid excessive handling or retraction of the foreskin until the injury has fully healed.

- Monitor for Infection: Watch for signs of infection, such as increased redness, swelling, discharge, or fever. If these occur, seek medical attention as soon as possible.

5. Healing Time and Future Risks
Healing time can vary based on the severity of the tear and how well you care for it. Generally, minor tears can take a few days to a couple of weeks to heal. Once healed, if you continue to experience tightness or discomfort, it may be worth consulting a healthcare provider about potential phimosis or other anatomical concerns.

6. When to Seek Medical Help
If you notice any of the following, it’s crucial to seek medical attention:
- Persistent pain or discomfort
- Signs of infection (increased redness, swelling, pus)
- Difficulty retracting the foreskin or painful erections
- Any new or worsening symptoms

Conclusion
In summary, managing a penile tear requires gentle care, proper hygiene, and monitoring for signs of infection. While it can be challenging to navigate healthcare in a foreign country, maintaining cleanliness and using appropriate topical treatments can significantly aid in healing. If symptoms persist or worsen, do not hesitate to seek medical help, as timely intervention can prevent complications.
