Probability of Pregnancy
Doctor, I would like to ask about my irregular menstruation.
My last period started on May 9th and lasted about 3-4 days.
I had unprotected intercourse from May 1st to the end of the month, except for May 16th when it was external ejaculation.
I did not get my period in mid-June and took a pregnancy test, which initially did not show an embryo.
However, during an ultrasound on July 2nd, it showed that I was 6 weeks and 6 days pregnant, with a due date of February 19th.
Later, during subsequent check-ups, the due date changed to February 16th.
I have two questions for the doctor: 1.
When did I conceive and implant? 2.
Could it be that I conceived and implanted from the external ejaculation on May 16th?

Dr. Cao Guotao reply Obstetrics and Gynecology
Hello, it is possible that conception occurred at the end of May or the beginning of June.
The chances of conception from external ejaculation on May 16 are quite low.
Wishing you good health.

Understanding conception timing and irregular menstrual cycles can be quite complex, especially when dealing with variations in menstrual cycles and the timing of ovulation. Based on the details you've provided, let's break down your situation and address your questions.
1. When Did You Conceive?
The timing of conception is typically calculated based on the first day of your last menstrual period (LMP). In your case, you mentioned that your last period started on May 9 and lasted for about 3-4 days. If we assume a typical 28-day cycle, ovulation would likely occur around May 23. Sperm can live inside the female reproductive tract for up to five days, which means that if you had intercourse around the time of ovulation, conception could have occurred shortly after that.
Given that you had unprotected intercourse on multiple occasions throughout May, including the 1st to the end of the month, it is possible that conception occurred from any of those encounters, but the most likely time would be around May 23. The ultrasound you had on July 2, which indicated a gestational age of 6 weeks and 6 days, aligns with a conception date around late May.
2. Could You Have Conceived from the May 16 Intercourse?
The likelihood of conceiving from the May 16 encounter, where you mentioned using the withdrawal method (体外射精), is significantly lower compared to unprotected intercourse. While the withdrawal method can reduce the chances of pregnancy, it is not foolproof, as pre-ejaculate fluid can contain sperm. However, given your timeline and the fact that you were already pregnant by the time you took the ultrasound, it is more probable that conception occurred from the earlier unprotected encounters.
It is also important to note that irregular menstrual cycles can complicate the estimation of ovulation and conception. If your cycles are longer or shorter than the average 28 days, this can shift the timing of ovulation, making it difficult to pinpoint the exact date of conception.
Additional Considerations:
- Irregular Cycles: If you have irregular menstrual cycles, it can be challenging to predict ovulation accurately. Tracking ovulation through methods such as basal body temperature, ovulation predictor kits, or monitoring cervical mucus can provide more insight into your fertile window.
- Ultrasound Dating: Ultrasound measurements are often more accurate than LMP for dating a pregnancy, especially in the first trimester. If your ultrasound indicated a gestational age that is ahead of your calculated due date based on LMP, it suggests that your baby is developing at a faster rate than average, which can happen for various reasons.
- Consulting Healthcare Providers: If you have concerns about your menstrual cycle, conception timing, or any other aspects of your pregnancy, it is always a good idea to discuss these with your healthcare provider. They can provide personalized advice and may suggest further evaluations if necessary.
In conclusion, while it is possible to conceive from any of the unprotected encounters in May, the most likely conception date aligns with your ovulation period around May 23. The May 16 encounter, given the method used, is less likely to have resulted in pregnancy. Understanding your cycle and working with your healthcare provider can help clarify any uncertainties you may have regarding conception and pregnancy progression.
