Flashes of light after retinal laser treatment?
Hello, Doctor.
I have high myopia of about -11.00 diopters and have regular fundus examinations every year.
In late January, I was diagnosed with a near rupture in the retina of my left eye, and there were grid-like changes in the peripheral area of my right eye.
I underwent retinal laser treatment for my left eye on February 14 and for my right eye on February 27 at National Taiwan University Hospital.
During the laser procedure, the doctor mentioned that the area in my right eye was quite large, so multiple laser applications were performed.
I have a follow-up appointment scheduled for May 22, three months later.
After the treatment on my left eye, there were no significant issues, but about three days after the treatment on my right eye, I noticed a small dark shadow in the upper part of my vision in a dimly lit room.
However, I couldn't see it clearly when I tried to focus on it.
Concerned about the possibility of retinal detachment, I went to an ophthalmology clinic on March 4 for a dilated examination.
The clinic indicated that the shadow was caused by vitreous traction on the retina.
That evening, I noticed flickering lights in the upper part of my vision in a dark room (which were less noticeable in bright areas), and when I closed my eyes, I saw a bright line in the upper right area.
On March 5, I went to the outpatient clinic at National Taiwan University Hospital for another dilated fundus examination.
The doctor stated that the problematic areas had been treated with laser and advised that as long as there were no new flashes or visual obstructions, I should return for a follow-up in three months.
I would like to ask the doctor the following questions:
1.
Can visual flickering occur after retinal laser treatment due to the healing process affecting the retina? This condition did not exist before the laser treatment.
How long might this flickering last? Given that I have already sought medical attention, under what circumstances should I seek care again?
2.
When I visited the clinic, the doctor mentioned that follow-up appointments after laser treatment are typically scheduled for 2-3 weeks later, but National Taiwan University scheduled mine for three months later.
Could this be a concern?
3.
I experienced coughing symptoms during the laser treatment.
Could this affect the outcome of the laser procedure?
I am very worried about these issues and appreciate your response.
Thank you!

Hello, 1.
Please return for a follow-up if there are any new changes.
(The doctor mentioned that the problematic areas have been surrounded with laser treatment, and if there are no new flashes or visual obstructions, follow the instructions to return in three months.) 2.
The follow-up appointment will be scheduled based on individual circumstances.
3.
No, it will not happen.
Wishing you safety and health.
Sincerely, Dr.

After undergoing retinal laser treatment, experiencing visual flashes can be concerning, especially for someone with a history of high myopia and retinal issues. Let’s address your questions and provide some insights into what you might be experiencing.
1. Visual Flashes Post-Laser Treatment: It is not uncommon for patients to experience visual disturbances, such as flashes of light, after retinal laser surgery. These flashes can occur due to the vitreous gel in the eye pulling on the retina, a phenomenon known as vitreous traction. This is particularly relevant for individuals with high myopia, as they are at a higher risk for retinal complications. The flashes you are seeing may be a result of this traction, especially since you mentioned that these symptoms began shortly after the laser treatment. Typically, these flashes may diminish over time as the eye heals and the vitreous gel stabilizes. However, if you notice any new symptoms, such as an increase in flashes, new floaters, or a shadow in your vision, it is crucial to seek immediate medical attention.
2. Follow-Up Appointment Timing: The follow-up schedule after laser treatment can vary based on individual circumstances. While some practitioners recommend a follow-up within 2-3 weeks to assess the healing process, others may feel that a longer interval is appropriate if the initial examination shows no immediate concerns. In your case, if your doctor at the hospital has assessed that the treated areas are stable and there are no new issues, a three-month follow-up may be acceptable. However, if you have ongoing concerns or if your symptoms worsen, it is always within your rights to request an earlier appointment for peace of mind.
3. Impact of Coughing During Laser Treatment: Generally, coughing during the procedure should not affect the outcome of the laser treatment. The laser is designed to target specific areas of the retina, and while any movement can potentially affect precision, the procedure is typically quick and the laser's effects are localized. If you have concerns about how your coughing may have impacted the treatment, discussing this with your ophthalmologist during your next visit can provide clarity.
In summary, while experiencing flashes after retinal laser treatment can be alarming, it is often a part of the healing process related to vitreous traction. Monitoring your symptoms and maintaining regular follow-ups with your eye care provider is essential. If you notice any changes or worsening of your symptoms, do not hesitate to seek medical advice sooner than your scheduled appointment. Your eye health is paramount, and proactive management is key to ensuring the best outcomes.
